Description
Overview: OSHA Region I requires a certified fire and explosion expert to perform the services described below as a fire and explosion expert in the review of the OSHA case file and on-site inspection of the Kleen Energy, Middletown, Connecticut gas explosion that killed six employees. Scope of Work: In performing the expert consultant duties, the expert will assist OSHA in the above noted case through research, review of OSHA documents, discussions with the applicable parties, preparation of reports, on-site investigation assistance and evaluation, and other applicable duties, as assigned by OSHA. This phase of the work is expected to commence on or about April 10, 2010. Statement of Work (May encompasses any or all of the following): •1. Review of OSHA case file. The expert will review the inspection files of the above noted case (or relevant portion thereof), other relevant documents in this case review. •2. Research and report. The expert will review relevant research reports from the open literature as well as other available material. A written summary with conclusions based on such research and on the review identified in #1 above will be provided to the Department of Labor-OSHA. •3. Discussions with applicable parties. The expert will discuss the issues in the case with the attorney of the Solicitor of Labor and OSHA personnel in person, by telephone and by mail. The expert will also be asked to participate in meetings and conferences related to the case. •4. On-site evaluation. The expert will conduct on-site investigation assistance for an evaluation associated with the OSHA inspection at the Kleen Energy Plant in Middletown, Connecticut. The expert would conduct an evaluation of the site to determine Cause and Origin. For this particular case the expert would review the evidence taken, conduct a scene evaluation, provide some gas calculations pertaining to the amount of gas used during the gas blows, identify and eliminate as many potential ignition sources within what ever area they determine to be the area of origin, conduct wind and weather evaluation as well as a review of witness statements. •5. The expert would develop a testing protocol for the assessment of the ground thaw heater. The assessment would be a two step process where the first step is an external examination of the heater doing non-destructive testing to determine the following: was the power unit for the heater energized. Is there identifiable electrical activity on the generator unit that would have supplied power to the boiler unit? Is there any evidence that the generator unit was operating in a natural gas enriched environment? Explain the condition of the boiler unit and the damage to its housing. Conduct testing to determine if the unit was in operation at the time of the explosion and determine if there is electrical activity to support that theory. •6. The expert would then develop a testing protocol for destructive testing of the ground thaw heater that will be developed and agreed upon by the interested parties. This may or may not be necessary if there is some way during the first part of the examination it can be determined with a degree of certainty that the unit was operational. The steps for this additional testing will be determined by what is found in the initial stage. 7. Lastly, the expert will need to develop and provide proper documentation regarding the findings and a report generated to support the views and opinions of the parties involved from your outside expert.
