- This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Dayton VA Medical Center (VISN 10) has been tasked with finding a Provider/Offeror in the following area in the state of OHIO: Dayton and surrounding communities, with experience and proven record of successfully working with residents recovering from co-morbid conditions suchas serious mental illnesses, substance abuse, disorders, and other serious medical conditions. The VAMC-Dayton, OH is seeking sources to assist with the provision of residential placement and supportive services through the Health Care for Homeless Veterans (HCHV) program. The primary goal is to develop alternative housing for the Veterans and to prepare them for independent living. The ultimate goal is the Veterans will eventually find their way to housing located within the community. To this end, Offerors must demonstrate ongoing capacity and resources to work with this population within a community setting. Interested parties may identify their interest and capability by submitting literature detailing their experience and resources. Offerors response shall be limited to 5 pages and shall include the Offerors Company Name, DUNS number, address, point of contact, title, phone number and email address. Offeror shall also identify their type of business (Small or large) and socioeconomic status whether Service Disabled Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman owned, Small Disadvantaged, etc. The NAICs code for this is 623220, Residential Mental Health and Substance Abuse Facilities (size standard of $7.0 million). Responses are to be sent electronically to ronald.smith@va.gov or mailed to Ronald Smith (NBC), 4100 West Third Street, Dayton, OH 45428-9000. Facsimile responses will not be accepted. After review of the responses to the sources sought notice, a solicitation announcement may be published on the FBO website. Responses to the sources sought notice are not considered adequate responses to the solicitation announcement. All interested parties will have to respond to the solicitation announcement in addition to responding to the sources sought notice. Interested parties are reminded to register with Central Contractor Registration (CCR) at http://www.ccr.gov in order to be eligible for award of Government contracts.
