- Sources sought to provide Diagnostic Test for the Boise VA Medical Center, (500 West Fort Street, Boise, ID 83702) VISN 20 is seeking businesses interested and capable of performing Diagnostic Test. SCOPE OF WORK. Contractor shall provide laboratory services to include pickup of specimens from the Boise VA Medical Center, Boise ID and transport of specimens to contractor's laboratory; preanalytic processing as defined in its laboratory users manual; analysis, reporting of analytic results, and consultation regarding selection, collection, transportation and result interpretation. -The contractor shall provide the following services: Provide all necessary supplies, not limited to the following: Requisition forms or agreed computer ordering system Specimen containers Dry ice and appropriate container Special instructions Current list of tests with reference ranges and specimen requirements, TAT, methodology Log books for samples sent out and results returned Specimen carriers All forms, including Chain of Custody forms Special media or tubes for viral samples All supplies for special tests (e.g., but not limited to, viral studies), including 24-hour urine containers with preservatives. Provide specimen pick up services as defined in the contract. Transport samples in such a manner as to ensure the integrity of the specimen. Contractor shall supply any special preservatives required for specimen preservation. Contractor shall notify VAMC of any specimen problems or discrepancies from the submitted manifest within 24 hours after shipping. Analyze samples. Routine test results shall be reported within the stated test TAT. If testing is delayed, notify Boise VAMC of the new estimated TAT. STAT test results shall be reported within 2 hours or less of specimen pick-up. Critical Value test results shall be reported immediately. Critical Valves will be called to BVAMC as soon as the test results are available. Consult with Laboratory on test results by telephone as needed. Provide BVAMC Laboratory with a means of communication to permit immediate inquiry regarding the status of pending tests, 24 hours per day, 7 days per week. Billing summaries shall begin the first day of the month and include the last day of the month. Tests referred to another laboratory shall be at no additional transfer charge or confirmation charge to the government. The Contractor shall provide to the VAMC the supplies and equipment within 1 week of receipt of the delivery order. The VAMC COTR will request replenishment of supplies from the Contractor on an as needed basis. SAMPLE PREPARATION. The VAMC will provide laboratory specimens prepared according to the contractor's laboratory users manual, identified, and labeled for testing. The contractor shall provide an adequate supply of requisition forms, specimen containers, dry ice and appropriate container, special instructions, and a current list of tests with reference ranges and specimen requirements. These requirements shall be defined in the laboratory user manual. The contractor shall be responsible for transporting and storing specimens for at least 7 days from the completed date of testing in such a manner as to ensure the integrity of the specimen where applicable. Contractor shall supply any special preservatives required for specimen preservation. The contractor courier service is responsible for packaging the specimens for transport. LICENSING AND ACCREDITATION.. Contractor shall have all licenses, permits, accreditation and certificates required by law. Laboratory Director shall be a licensed American Board of Pathology certified pathologist or appropriately certified bio-analyst. Contractor policies and procedures must comply with the Health Insurance Portability and Accountability Act (HIPPA). All medical facilities providing laboratory services under the contract must possess a valid state license and meet JC standards as well as CLIA requirements and standards of the College of American Pathologists (CAP). SERVICE. The contractor shall provide telephone number(s) and contact person to be used by the VAMC to make specimen problem inquiries and problem solving at all times including weekends and holidays. The contractor shall include names and telephone numbers of technical Directors and Pathologists available for consultation. Contractor agrees to maintain the minimum acceptable service, reporting systems and quality control. Contractor shall advise facility of any changes in methodology, procedure, reference ranges delays in testing, exceeding the maximum TAT and any new tests introduced. Exception handling: Contractor will notify VAMC, Laboratory Service within 12 hours of any problems with specimens received. TESTING METHODOLOGY, REFERENCE AND TURN AROUND TIME. Testing methodology and reference ranges for a test must be defined in the laboratory user manual. Contractor to provide VAMC copies of updated test reference manuals as updates occur but no less often than yearly. PERSONNEL. The Contractor shall make sure employees have current and valid professional certifications before starting work under this contract. Technologist, medical technicians, and cytotechnologist shall meet personnel qualifications required by Clinical Laboratory Improvement Act (CLIA) '88 Guidelines. HOURS OF OPERATION. Normal Hours of Operation. The Contractor shall perform the services required under this contract 24 hours a day, 7 days a week, 12 months of the year. The Contractor shall provide routine scheduled specimen pickup at a time mutually agreed upon by the VAMC COTR. PATIENT LISTS. The Contractor shall treat patient lists, no matter how developed, as privileged information. Lists and/or names of patients shall not be disclosed to or revealed in any way for any use outside the participating VAMC without prior written permission by the VAMC COTR or Chief of the Medical Staff. SPECIMEN PICK-UP TIMES. The Contractor shall provide routine scheduled specimen pickup at a time mutually agreed upon by the VAMC COTR but not less than once each day. The VAMC COTR or designee shall notify the Contractor during weekends and federal holidays, via telephone, when a pick-up courier is required. Contractor and BVAMC shall have an agreement to non-pick up holidays. LIST of TEST. See attached listing for list of test required by Boise VAMC Please respond via e-mail ONLY. Also state if you currently have a Federal Supply Schedule Contract or a VA contract. Only this minimal information is requested. A Request for Proposals shall be issued at a later date. The NAICS code for the procurement is 541380, Testing Laboratories, and the small business size standard is $12 million.
