- Performance Work Statement (PWS) FA4460-10-T-0040 Solicitation (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Part 12 Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and quotes are being requested. (ii)This solicitation, FA4460-10-T-0040, shall serve as a Request for Quotation (RFQ). (iii)This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-39 (Effective 19 Mar 2010 [FAR obtained from 554 ELSG/KSL]). (iv)This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 711130 and the small business size standard is $7.0 million. (v-vi)The following commercial item(s) is/are requested in this solicitation; Base Period: CLIN 0001 - Direct choir/congregation in musical worship during the 0800 Liturgical service. Work shall be performed IAW PWS. POP: 15 APR 2010 - 30 SEP 2010 CLIN 0002 - Direct choir/congregation in musical worship during the 1100 Traditional service. Work shall be performed IAW PWS. POP: 15 APR 2010 - 30 SEP 2010 CLIN 0003 - Direct choir/congregation in musical worship during the 1800 Contemporary service. Work shall be performed IAW PWS. POP: 15 APR 2010 - 30 SEP 2010 CLIN 0004 - Provide music, direct choir members, direct instrumentalists during weekly music rehearsals, and coordinate musical requirements with the service chaplain. One unit/hour of practice per service. Work shall be performed IAW PWS. POP: 15 APR 2010 - 30 SEP 2010 CLIN 0005 - Coordinate with the Sr. Protestant Chaplain quarterly on requirements and yearly progress. Work shall be performed IAW PWS. POP: 15 APR 2010 - 30 SEP 2010 CLIN 0006 - Annually review music library. Work shall be performed IAW PWS. POP: 15 APR 2010 - 30 SEP 2010 Option Year 1 Option Year 2 Option Year 3 (vii)The contractor shall complete the base period of performance from 15 Apr 2010 to 30 September 2010, FOB: Destination for delivery to Little Rock AFB, AR 72099. (viii-xiii) The provisions and clauses listed in the attached SF 1449 Solicitation FA4460-10-T-0040 apply to this acquisition. The Government reserves the right to award on a multiple award or an all or none basis. All firms must be registered in the Central Contractor Registration database at www.ccr.gov in order to be considered for award. It is recommended that responding offerors register on ORCA (http: //orca.bpn.gov) / ensure ORCA is current. FAR 52.204-8 mandates the use of ORCA on or after 01 Jan 2005. (xiv)(RESERVED) (xv) Quotes are required to be received NO LATER THAN 1:00PM CST, Friday, 09 Apr 2009. (xvi) All quotes must be sent to the attention of A1C Lucille D.
