- Denatal Service Pricing Dental Exams Services for Dental Examinations Statement of Need 914 Airlift Wing / ASTS 2560 Kirkbridge Dr. Bldg 802 Niagara Falls Air Reserve Station, NY 14304 RFQ FA6670-10-Q-0014 07 April 2010 Dental Services Project: SERVICES NONPERSONAL: Contractor to provide all plant, labor, material, parts, equipment & transportation necessary to provide the following: " As needed" Services for a Civilian Dentist to complete dental exams at the Niagara Falls Air Reserve Station, Base Medical Clinic primarily on Saturday's of the Unit Training Assembly ( UTA) weekend from the period of 07:15 AM to 4:15 PM. Unit Training Assembly weekends are usually scheduled for the first weekend of each month to assure dental readiness. Maybe required " As Needed" for services on Sunday of the Unit Training Assembly weekend or if needed for a Unit deployment processing clearance. Objective: Is to maintain the high level of dental readiness that has already be obtained and not fall below the 22nd Air Force's mandated requirement of 95% class 1 & 2 and less than 5% class 4. Expectations: The minimum expectations of the contract civilian dentist are as follows: 1. Complete periodic dental exams on 914 Airlift Wing members as required by regulations. 2. Review health history, complete required section of form and sign in accordance with regulations ( AF Form 6960. 3. Request and review radiographs as prescribed by the American Dental Association. 4. Complete dental exam on members (patient) by dictating findings to dental assistant for charting findings into dental record. 5. Proper dental classification of member (patients) 6. Verify charting and documentation in dental record for accuracy and sign entries (AF Form 623(a). 7. Follow and maintain infection guidelines. 8. Maintain a valid State dental license. 9. Continuing education requirement maintained. 10. Professional role model for dental stall, Airlift Wing members and patient population. Periodic Dental Examination: A.D.A define as "periodic oral exam" as am exam that is done on established patients to determine any changes in dental and health status since a previous comprehensive or periodic evaluation pof tooth wear, functional occlusal contacts, and perceived overall quality of oral health. A periodic examination by a professional is an effective means of averting the Serious sequelae, which may develop due to failure to treat these lesions ar the appropriate time. It is therefore justified, to advise that continued regular routine dental check-ups when nothing unusual is found.
