- ***SOURCES SOUGHT ONLY*** SCOPE OF WORK Seeking interested Contractor(s) to provide Purchased Skilled Home Care Services to Beneficiaries of the Department of Veterans Affairs (hereinafter called the VA), in the El Paso VA Healthcare System catchment area at a per visit rate. The El Paso VA Health Care System serves veterans in far Southwest Texas and Dona Ana County, New Mexico. The VAHCS provides primary and specialized ambulatory care services at its main campus the El Paso VA Health Care Center (VAHCS) with consultants and fee- basis specialists supplementing the VAHCS medical staff. Purchased Skilled Home Care Services and Therapies include, but are not limited to: 1. Skilled Nursing (est. 4,000 visits/yr) 2. Physical Therapy (est. 6,000 visits/yr) 3. Speech Therapy (est. 12,000 visits/yr) 4. Occupational Therapy (est. 1,500 visits/yr) 5. Medical Social Services (est. 6,000 visits/yr) Contractor(s) shall be required to provide all services listed herein. The quantities identified herein are estimates only. The VA does not guarantee or imply that any minimum number of orders or referrals will be placed or made. Supplies: Medical supplies may be furnished by the home care agency only if the supplies needed to provide care are not available in the veteran's home. The Contractor must make every effort to obtain supplies from the VA prior to a commercial purchase. Authorization to provide the supplies must be obtained from the VA Home Care Coordinator, if provided during regular VA business hours; 8:00 AM o 4:30PM, excluding weekends and holidays or as soon as possible thereafter. Supplies are to be billed at the VA at average wholesale price (AWP) + 3% or wholesale + 3% with a copy of the receipt attached. An itemized list of the supplies provided must accompany each claim. If the home care agency is required to provide supplies for greater than one week, the VA Home Care Coordinator must be notified.
