- Statement of Work (SOW) The Uniformed Services University (USU), located on the National Naval Medical Campus in Bethesda, MD requires: professional, comprehensive, and confidential counseling and referral services for the University's employees for a wide range of personal problems (e.g., alcohol abuse, drug abuse, financial, personal, emotional, psychological, marital, family, legal, stress related, etc.); the prevention and resolving of employees drug use through active participation in the USU Civilian Employee Drug Testing Program; counseling and referral services to families of USU employees with personal problems and to USU employees whose family members have personal problems; on-going publicity/promotion of the employee assistance program and services; employee (both supervisory and non-supervisory) orientation, education, and training relative to the employee assistance program and services; statistical data gathering and analysis sufficient to satisfy Agency reporting requirements; and an end-of the contract year cost/benefit analysis. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. It is the responsibility of the contractor to monitor FedBizOpps for any amendments to the solicitation. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular 05-38. This solicitation is set-aside for small businesses and the applicable NAICS code for this requirement is 624190 - Other Individual and Family Services with a size standard of $7.0 million. This requirement is for a firm fixed priced contract for the following Contract Line Item Number (CLIN): Price : The contractor should provide pricing as set up below (Contract Line Number (CLIN), description, monthly amount for the base year and four option years. CLIN 0001 Employee Assistance Program (EAP) - BASE YEAR The contractor shall provide all professional, confidential counseling, and referral services for federal civilian employees for a wide range of personal problems in accordance with the terms and conditions as outlined in the statement of work. u/i: 12 months. CLIN 0002 Employee Assistance Program (EAP) - Option Year I The contractor shall provide all professional, confidential counseling, and referral services for federal civilian employees for a wide range of personal problems in accordance with the terms and conditions as outlined in the statement of work. u/i: 12 months. CLIN 0003 Employee Assistance Program (EAP) - Option Year II The contractor shall provide all professional, confidential counseling, and referral services for federal civilian employees for a wide range of personal problems in accordance with the terms and conditions as outlined in the statement of work. u/i: 12 months. confidential counseling and referral services for federal civilian employees for a wide range of personal problems in accordance with the terms and conditions as outlined in the statement of work. u/i: 12 months. CLIN 0004 Employee Assistance Program (EAP) - OPTION YEAR III The contractor shall provide all professional, confidential counseling, and referral services for federal civilian employees for a wide range of personal problems in accordance with the terms and conditions as outlined in the statement of work. u/i: 12 months. CLIN 0005 Employee Assistance Program (EAP) - OPTION YEAR IV The contractor shall provide all professional, confidential counseling, and referral services for federal civilian employees for a wide range of personal problems in accordance with the terms and conditions as outlined in the statement of work. u/i: 12 months. The statement of work is an attachment to this synopsis. Evaluation Factors The government will award a firm-fixed-price (FFP) contract resulting from this solicitation to the responsible offeror whose offer conforms to the combined synopsis/solicitation, and is most advantageous to the government, based on price and the technical factors listed below. To be eligible for contract award, a contractor must be register with the Central Contractor Registration (CCR) database. For instructions on registering with CCR, please see CCR website at www.ccr.gov. Technical •1. Educational and experience qualifications. Your quote shall include educational and experience background of individuals offered under this contract. Your response shall present the experience of individuals offered in the various professions. Identify educational degrees and professional positions held, and length of service held in those professions. Specifically, each item in the following list shall be reflected in the education and/or experience qualifications of at least one individual offered for service on this contract: a. Confidential assessment, counseling, and referral services for a wide variety of issues including but not limited to alcohol and drug abuse, emotional, financial, legal, marital, child and health problems. Counselors must be certified in alcohol and drug abuse counseling. b. Experience providing critical emergency stress debriefings and employee counseling. c. Experience providing counseling and training in accordance the Federal Drug-Free Workplace Program (Executive Order 12564). d. Experience providing conflict resolution. e. Experience providing on/off site personalized counseling. •2. Technical Approach. Describe your ability to organize, schedule, and staff the contract to provide the required services. •3. Past Performance. Present work accomplished by offeror on services with similar tasks to those identified in the statement of work. Past performance shall include a description of services where individuals provided the required services offered in this synopsis. Identify customer references listing contact information: address, e-mail address and phone number. and Price. Technical acceptability will be determined solely on the content and merit of the submitted in response to the provision as it compares to the minimum characteristics provided above. Therefore, it is essential for the offerors provide sufficient technical literature, documentation, etc., in order for the Government evaluation team to make an adequate technical assessment of the proposal as meeting technical acceptability.
