- The Oklahoma City Area Indian Health Service in accordance with Far Part 12, Acquisition of Commercial Items" and Part 15, Contracting by Negotiation, has a requirement for an Indefinite Delivery/ Requirements Contract to provide reference laboratory and anatomical pathology testing services for 12 healthcare facilities in the Oklahoma City Area. The successful contractor shall provide reference laboratory, cytology, histology and anatomical pathology testing services required by the healthcare facilities service units of the Oklahoma City Area office. The locations include Claremore Indian Hospital, Clinton Indian Health Center, El Reno Indian Health Center, Watonga Indian Health Center, Lawton Indian Hospital, Anadarko Indian Health Center, Carnegie Indian Health Center, Pawnee Indian Center, Pawhuska Indian Center, Haskell Indian Health Center, White Cloud Indian Health Center, and Wewoka Indian Health Center. The relative number of specimens submitted by each location is variable and dependent upon the size of each facility. For the purpose of evaluation the quantities of each requirement will be estimated from historical data. The prospective contract shall be a base year beginning September 1, 2010 through August 31, 2011, with provisions for four one year options. Solicitations will be available on or about April 26, 2010. Receipt of Offerors will be on or about May 25, 2010. The North American Industry Classification System Code for this solicitation is 621111. All prospective offerors desiring to submit proposals in response to any Federal Agency Request for Proposal must obtain a Dun and Bradstreet number at www.dnb.com. There is on cost to obtain this number. Offerors must also register at the Central Contractor Registration site at www.ccr.gov.
