- Statement of Work for Technical Consultation for the ESRD Program Request for Quotation Technical Clinical and Coverage Consultation for the End Stage Renal Disease (ESRD) Program #221-0-3086-06 This is a Request for Quotation. A quote is being requested and a written solicitation will not be issued. Requirement number 221-0-3086-06 is issued as a Request for Quotation. The Centers for Medicare and Medicaid Services (CMS), Survey and Certification Group, has a requirement for a contractor to provide technical consultation in the clinical psychosocial areas and the coverage-dependent areas of the new regulations related to the ESRD Program in order to implement regulatory program changes and to monitor the impact of that implementation on Medicare beneficiaries. This requirement will include assistance with the ongoing analysis and interpretation of technical areas of the new ESRD regulations, including the psychosocial areas and coverage-dependent areas; development and implementation of ESRD survey protocols and tools; development and implementation of ESRD training programs with associated expertise in converting training materials so that they are both 508 compliant and Web site accessible; consultation on issues related to psychosocial clinical practices in dialysis facilities; and assistance with updating a revised automated survey system, i.e., the STAR program. Detail information is provided in the attached Statement of Work and the Deliverables. The period of performance will be for 12 months, approximately July1, 2010 through June 30, 2011. The government estimates a level of effort for this requirement is approximately 1,872 professional person hours. This will be awarded as a firm fixed price procurement using simplified acquisition procedures (threshold $100,000). Your response must be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov or in paper no later than 11:00am on May 7, 2010 to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd, Mailstop C2-21-15, Baltimore, MD 21244. Your response shall be no more than 12 pages and shall include the following: 1) A brief overview of the contractor's capabilities; 2) Past performance information (relevant experience to perform the required services); and 3) Price of the deliverables (to include hourly rates). Your response will be evaluated on a best value basis using the following: 1) attached Technical evaluation criteria 2) Personnel 3) Past performance and 4) Price of the deliverables. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price; the best value (considering price and other factors). All quotes/responses submitted shall remain valid for a period of 180 days. Offerors are advised that CMS is a secured facility and therefore, offerors should allow time to obtain the appropriate security clearances (e.g., parking pass, visitor badge, etc...) Vendors are also advised that the CMS mailroom accepts only U.S. Postal Service mail. When utilizing an overnight carrier, you are cautioned that you must allow time for the proposal to be delivered to the exact mail stop noted above by the carrier by the time specified in this RFQ. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work indentified in the Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated.
