Description
PERFORMANCE WORK STATEMENT (PWS) 1.0 INTRODUCTION The 633d Logistics Readiness Squadron, Vehicle Management Flight has a requirement to provide and maintain five (5) protective coveralls for each assigned civilian and military employee within vehicle maintenance. 2.0 SCOPE The purpose of this requirement is to protect civilian and military personnel's uniforms while performing duties related to vehicle maintenance mechanics. 3.0 PERFORMANCE WORK STATEMENT The contractor will obtain appropriate sizes from the 633d Logistics Readiness Squadron, Vehicle Management Flight, Points of Contact (POCs). The contractor will provide five (5) new coveralls per assigned individuals at the beginning of the contract period for initial issue. Additional coveralls for newly assigned personnel will be provided within 14 days and as personnel are reassigned or are no longer required they will be deleted from the contract and contractor's database. The contractor will provide a pickup and delivery service at normal business hours (0700 to 1600 Monday to Friday) to the Vehicle Management Flight, Materiel Control Section at 61 Elm Street, Langley AFB, Virginia 23665. The contractor will provide a weekly cleaning service along with pickup of dirty coveralls on a weekly basis. The contractor will repair coveralls as needed when identified in accordance with commercial standards and military safety standards or replace coveralls as necessary to maintain a safe, clean, neat, and professional image. The contractor will provide 633d LRS Vehicle Management Flight with a tracking system for all coveralls being turned in for cleaning. The contract will not access any start up fees to the contract. If the number of coveralls originally covered changes plus or minus, it is the contractor's responsibility to notify the Vehicle Maintenance Superintendent or designated POC and the Contracting Office of any needs to adjust the monthly cost. The contactor agrees to monthly or quarterly payments. Description of Coveralls: The coveralls will be dark blue in color, long sleeve and comply with all Air Force Occupational Safety and Health Standards. Each coverall will have a white name tag (size no larger than 2" X 4") of each individual on the right side with the appropriate name appearing in blue scripted letters. 4.0 PERIOD OF PERFROMANCE The contract period will begin on an annual basis with 4-year option year for continue service. The first optional period will begin 1 Oct 2010. 5.0 FUNDING It is the contractor's responsibility to notify the Vehicle Management Flight Manager or designated POC and the Contracting Officer of any needed adjustments to the monthly cost (if the number of coveralls changes plus or minus this is understandable but if the cost preset changes per week the contractor will need to contact the above mentioned personnel). 6.0 PAYMENT PROCEDURES Request payment is through DFAS limestone and will be submitted in Wide Area Work Flow (WAWF) to complete monthly or quarterly invoices to ensure payments are made on time. AF Form 9 will be provided to the 633d Contracting Squadron to reserve funds for the coverall service. 7.0 POINTS OF CONTACT Mr. James E.
