Description
This is a Sources Sought Notice (not a pre-solicitation notice pursuant to FAR Part 5) to identify potential sources that can provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to perform non-personal services to conduct patient surveys. This notice is for market research purposes and requests interested sources to provide their companys capability information ONLY. The Center for Health Care Contracting, Fort Sam Houston, Texas, seeks interested sources with the required capabilities to provide patient survey services. This initiative is for the Office of The Surgeon General (OTSG), Army, which operates Military Treatment Facilities (MTFs) in both the continental United States (CONUS) and Overseas locations (OCONUS). The types of surveys to be used are Provider Level Patient Satisfaction Survey, Warrior in Transition Unit (WTU) Survey, Benchmark Survey and Provider Survey. The offeror would be required to provide qualified personnel, services, materials, equipment, supplies and facilities necessary to perform required services. This survey is to determine the capability of potential sources and the availability of commercial sources and services pursuant to FAR Part 10. This notice is for informational purposes only and does not constitute an Invitation for Bid nor Request for Proposal, and is not to be construed as a commitment by the Government. The Government will not reimburse contractors for any cost incurred for their participation in this survey. It is anticipated that any resultant contract that might result from this survey would be Firm Fixed priced. Period of performance would be a base plus four 1 year options. The North American Industrial Classification System (NAICS) code is 541910; and the small business size standard is $7.0 MIL. Responses should contain Company Name, Address, Point of Contact, Cage Code, Phone Number, FAX Number, Size of Business pursuant to NAICS code 541910 (size standard $7,000,000), GSA contract number if applicable, a brief resume of performance of similar services (since January 2006) of the magnitude described above (include contact and phone number of projects listed), a statement as to whether your company is domestic or foreign owned. Large and small businesses are encouraged to participate in this survey. In addition to the size of business, indicate all classes of business(es) that fit your firm; i.e. Service Disabled Veteran Owned, Small Disadvantaged, Veteran-Owned, Woman-Owned, etc. Additionally, provide answers to the following: What are industry standards for collecting surveys for this type of service? How do you obtain surveys from customers (i.e., mail, telephone, e-mail, etc.) Interested sources should submit capabilities information or executive summary (limited to 5 pages) to Liza.Esmond@amedd.army.mil via e-mail. Any questions regarding this requirement should be submitted in writing to Liza Esmond, Contract Specialist, at Liza.Esmond@amedd.army.mil or Johnie Johnson, Contracting Officer, Johnie.Johnson@amedd.army.mil prior to the close of this notice. Responses are due by 5 May 2010 at 2pm. Please provide detailed answers to the following questions. (1) Does your company have experience collecting data through mailed surveys, Interactive Voice Response (IVR) and website? Provide description of any other source of collection. If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (2) Does your company have experience providing a user friendly database for sample and response data? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (3) Does your company have experience collecting response rates and providing results in a user friendly format? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (4) Does your company have experience creating databases that provide survey responses and corresponding format files that are user friendly and easy for the customer to manipulate for required inquiry of results? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (5) Does your company have experience providing and re-coding variables? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (6) Does your company have experience creating and providing reports and technical documentation, in a timely manner and in a clear and concise format? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (7) Does your company have experience creating a user friendly website that customers and patients can use to take surveys and retrieve and review data and reports? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (8) Does your company have experience conducting statistical analysis of results? Describe the education level and employee mix of employees who conduct this type of analysis. If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (9) Does your company have experience providing system inter-face with Government, local, state or federal entities, or major healthcare facility or hospital? If so, give a brief example of your experience and the magnitude (price and scope) of your experience. (10)Does your company have the facilities to support a major survey requirement? If so, briefly describe your facility set up. (11) Has your company ever tried to get its IT system DIACAP or DITSCAP accreditation? If so, did your companys current system receive accreditation? When and by which agency? If not, when, which agency and was your company able to resolve the issues. (12) Please indicate the magnitude of the largest contract your have held, that dealt directly with Patient Satisfaction Surveys and requires statistical analysis; to include base and any option years.
