Description
The purpose of this Amendment 0003 to RFP W25G1V-10-R-0029 is to make a change to Attachment 1, Statement of Work and answer offeror questions received. The response date for the solicitation is extended to 30 APRIL 2010 @ 3:00 PM local. 1. Reference combined Synopsis/Solicitation W25G1V-10-R-0029, Attachment 1, Paragraph 4, ADD: The waste blast media contains cadmium, chronium and lead. Personal Protective Equipment (PPE) for these hazards should be used. Offeror question received with the Governments response: Q1. What are the specific hazardous materials we will be dealing with? This will insure we provide the proper PPE equipment for our contractors? A1. The waste blast media contains cadmium, chromium and lead. PPE for these hazards should be used. See Attachment 1, Paragraph 4, as per this amendment, Paragraph 1. 2. Reference combined Synopsis/Solicitation W25G1V-10-R-0029, Attachment 1, Paragraph 7, ADD: The raw steel flooring material will be cleaned by Tobyhanna Army Depot before disposal by the government. Offeror question received with the Governments response: Q2. Will the raw steel material to be removed require additional cleaning before placing this material in the supplied roll off scrap bins? If so, is there an area near this building to perform this cleaning? Or will this require a portable room to be built for the cleaning of this material? A2. The raw steel material to be removed will be cleaned by Tobyhanna Army Depot before disposal by the government. See Attachment 1, Paragraph 7, as per this amendment Paragraph 2. 3. Additional offeror questions received with the Governments response: Q3. We are requesting a MSDS sheet on all material located in this room prior to any work being performed along with a swab test to insure there is not any other hazardous materials to be concerned with. A3. Swab test is not required. Hazards are those listed in Paragraph 1 of this amendment. For MSDS sheet on the blast media, contractor can contact Norton Abrasives. The media type is Zirconia Alumnia, MCA-1360. Q4. During the removal of the flooring will it be required to build a temporary room around this blast room to insure NO dust particles escape to atmosphere? A4. No temporary room is required. The work should be confined to the blast booth. The blast booth doors can be shut to prevent dust particles from escaping. Q5. Can parts and materials be stored near this blast room during the removal and installation and left on-site until this project is completed? A5. Per combined Synopsis/Solicitation, Attachment 1, Paragraph 8, removal and installation is to be done within six (6) days and in one mobilization. The blast booth will be turned over to the contractor for the six (6) days. Parts and materials can be kept near the blast booth, however, the government is not responsible for their safe keeping. Q6. Is there compressed air near the facility for the required breathing apparatus for the technicians? A6. Yes, there are breathing air lines near the blast booth. Q7. Is there suitable power near or in the room for the power of welders, plasma cutters, grinders, etc.? If so, what is available and are there disconnects for this type of equipment? If not, will power be provided before the project starts? A7. Within 30 feet of the blast booth doors there is a welding receptacle rated at 460 volts; 60 Amp receptacle type is a Hubbel #HBL460RS2W, 3P4W, 60 AMP. 110V receptacles are also near. Q8. Due to the hazardous materials in this room, is there a clean room with showers for the contractors and also an area to dispose of any mask or suits on the base that the contractors will have access to? A8. Disposal of masks and suits is the responsibility of the contractor. The contractor will have access to the showers in a rest room near by if the contractor chooses to use them. Q9. Will there be a point of contact for the duration of this project to discuss issues and concerns? A9. All issues and concerns shall be directed to the Contracting Officers Representative (COR). Q10. Is there any part of this removal or installation considered confined space? A10. No part of this removal or installation is considered confined space. Q11. Can service trucks be parked near this room for ease of tools needed if they are gas vehicles? A11. No, all vehicles must be parked outside. Q12. Will the COR provide a completed Form 1449 with the specific contract information blocks filled in? A12. Per combined Synopsis/Solicitation, Proposal Requirements, the offeror shall complete a Stanard Form (SF) 1449 (available at http://www.forms.gov/), blocks 5, 12, 17a, 17b (as appropriate), 19 thru 24, 30a, b, and c. This is the contractors signed offer. 4. The offer due date is extended. Delete: 27 APRIL 2010 @ 4:00 P.M.; Add: 30 APRIL 2010 @ 3:00 P.M. 5. All other terms, conditions, and requirements of the solicitation remain the same.
