Description
**REQUEST FOR INFORMATION**This is a request for information only. The Clement J. Zablocki VA Medical Center has a requirement for Environmental (Hazardous Substance Identification) and Infection Control Sampling, Testing, and Monitoring work. Work encompasses a variety of environmental samplings, inspections, and Professional Industrial Hygienist services including: 1.IH/Environmental on-site work 2.PH/Environmental on-site work 3.CIH Review 4.IAQ/Environmental report preparation 5.Project management 6.Air sampling: a.Noise b.Photoelectric c.Xylene d.Glutaralehyde e.Nitrous Oxide f.Toluene g.Methyl Methacrylate h.Mercury i.Methanol j.Ethylene oxide k.Formaldehyde l.Isopropyl Alcohol m.Acetic Acid 7.Fungi & Bacteria Sampling: a.Air, Swab, Surface, Bulk Solid & Liquid b.Volitile organic compounds c.Respirable dust 8.Soil/water/misc. sampling: a.Silver (by ICP) b.BTEX c.PNA's d.RCRA Metals e.LUST Priority pollutants f.Volitiles g.Lead h.TLCP lead 9.Infectious Control Inspections The medical center has a variety of construction projects progressing at all times and requires the above sampling, testing and monitoring in support of those projects. This requirement will be filled by a single contractor. This is not a request for proposal. This is a Request for Information to determine sufficient competition for required VA set-asides for the Service-Disabled, Veteran-Owned Small Business (SDVOSB) socioeconomic group. QUALIFICATIONS Only SDVOSB contractors who are able to respond to emergent situations at the Clement J. Zablocki VA Medical Center, Milwaukee, Wisconsin within 4 hours of notification need respond. MINIMUM QUALIFICATIONS The contractor must have had an established business for at least five (5) years with an office and full time staff, including, at least, one (I) qualified Professional Industrial Hygienist (PIH), one (1) qualified Industrial Hygienist (IH), and the necessary support personnel to perform the requirements of this solicitation. A PIH must meet the minimum qualifications listed below. An IH must met the minimum bachelor degree in related science; have a minimum of the training courses on asbestos abatement and monitoring, and have a minimum of two (2) years field experience with all aspects of asbestos sampling and asbestos abatement monitoring and inspection. The contractor shall obtain all necessary licenses and/or permits required to perform this work and provide them to the Contracting Officer prior to award. Failure to submit the requested documentation maybe grounds for cancellation/or termination. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract, that is caused by the contractor's employee's fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of Wisconsin. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder, with regard to any claims, loss, damage, injury and liability resulting there from. The PIH (Professional Industrial Hygienist) shall meet the following minimum qualifications: a. Meets OSHA's definition of a "competent person" under 29 CFR 1926.58(b). b. Meets the American Industrial Association's definition of a "professional" IH, with a minimum of one (1) year of the five (5) years required experience requirement being related to asbestos work. c. Has experience with IAQ/Environmental projects of similar complexity and size. d. Has experience with programs involving service maintenance personnel. e. Has completed formal training in respiratory protection, waste disposal, project management, and performing environmental assessments. f. Provide support staff and backup capacity to provided adequate level of support throughout the performance of contract work. g. PIH must have performed a minimum of two (2) environmental assessments, two (2) infection control assessments in a hospital environment, assisted A/E's in the preparation of two (2) abatement designs, performed abatement construction monitoring, for a minimum of two (2) projects, conducted the final inspection and clearance testing for a least two (2) projects. h. Laboratory testing for PMC's must be done by an AIHGA accredited laboratory. Bulk sampling should be done by a NVLAP (National Voluntary Laboratory Accreditation Program) approved laboratory.
