- Description
- The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Service Requirements type contract for pediatric medical services at the Fort Thompson Indian Health Center in Fort Thompson, South Dakota. The Period of Performance will be from the Date of Award through 1/31/2011. The Contractor will work up to 2 days per week; with a tour of duty from 8:00 AM to 4:30 PM. The services needed include consultation, treatment, and/or ambulatory surgical procedures which can be done at the health center. Individual task orders will be issued against the Requirements type contract for services as established in the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-117, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-10-117 and provide a postal mailing address or email address and state preference as to how the RFQ package should be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be Friday, April 12, 2010. The closing date of the RFQ package is April 26, 2010. THIS REQUEST IS FOR FULL AND OPEN COMPETITION. THE DUE DATE FOR RFQ-10-117 HAS BEEN EXTENDED TO MONDAY, MAY 5, 2010 AT 5:00pm CST.
