- Sole Source Letter This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4530105A002 this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-40 effective 22 APR 2010; Defense DCN 20100408 effective 08 Apr 2010, and AFAC 2010-0402 effective 02 Apr 2010. This acquisition is unrestricted; the North American Industry Classification System (NAICS) code is 611710. The business size standard is $7 M. The Federal Supply Class (FSC) is U010. The Standard Industrial Classification (SIC) is 8299. Travis AFB has a requirement for 60th MDG to maintain accreditation with Accreditation Council for Graduate Medical Education (ACGME) to sponsor medical residencies IAW AFI 41-117. This requirement is sole sourced to: Accreditation Council for Graduate Medical Education. 515 North State Street Suite 2000, Chicago, Illinois 60654-4865. The 60th MDG requires accreditation or they will not be able to continue with their graduate medical education training programs. Services Nonpersonal. Period of Performance 01 Jul 2010 - 30 Jun 2011. 0001-AA : Accreditation program fee for more than 5 residents in family medicine clinic. 1 Each 0002-AA : Accreditation program fee for more than 5 residents in diagnostic radiology. 1 Each 0003-AA : Accreditation program fee for five or fewer residents for transitional year. 1 Each Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition.
