Description
Contracted Dentist for the Kittrell Job Corps Center A. PERIOD OF PERFORMANCE The period of performance for this agreement will be from June 1, 2010 through May 31, 2011. B. SCOPE OF DENTAL SERVICES 1. The service of the Professional or designee will be made available to the Center, as follows: a) Oral examinations, emergency treatment and routine dental care. 2. All routine care will be performed at the Center Wellness Center. Specific hours for, ration and dental procedures will be mutually agreed upon by the Professional and the Center Director or designee. Any third party payments will be credited to the Center; and the services to be provided will include, but not be limited to, the type needed in accordance with the priority system listed in Job Corps Technical Assistance Guide (TAG 6-A), Dental Program. These include provision of the following: a) Provision of a routine dental examination of the American Dental Association Classification, Type II, between the 45th and 75th day after the enrollee's arrival on Center. b) Provision of a written diagnosis and treatment plan for each enrollee. On the basis of diagnosis, priorities for the receipt of dental care will be established using the Job Corps priority classification system. c) Provision of routine dental care, as defined by the Job Corps Technical Assistance Guide (TAG 6-A), Dental Programs, as approved by the Center. d) Provision of dental hygiene services and preventive dental health procedures, e.g., toothbrushing instruction, which will promote the oral hygiene of all enrollees. e) Provision of an appropriate referral system to ensure the receipt of special care, as defined by Technical Assistance Guide (TAG 6-A). f) Provision for coverage by a qualified substitute, approved by the Center, during the absence for the Professional. g) Provision for twenty-four (24) hour emergency coverage provided that the professional is reasonably available to provide consultation. h) A diagnosis and treatment plan for each Job Corps student within two (2) weeks after completion of routine dental examination. On the basis of the diagnosis, priorities for the receipt of dental care will be established. (The plan should include a listing of the teeth involved and specific services to be rendered.) 3. The development, implementation, and monitoring of the total Center Dental Health Program to include, but not be limited to: a) Advice to the Center Director, Medical Director and Wellness Staff in all dental or dental-related matters. This shall include the advisability of terminations for dental reasons, the cost of services, and any other areas deemed appropriate. b) Provision for assistance in developing the coordination and integration of the Dental Health Program with the other health components and other Center activity. c) Insurance of the maintenance and confidentiality of all required dental records. d) Provision of assistance in the development and implementation of the dental education component of the Health Education Program. e) Identification and utilization of the most appropriate and least expensive sources for obtaining necessary dental pharmaceutical supplies. f) Insurance and maintenance of signed and dated Standing Orders (annually) for dental care in accordance with the Policy and Requirements Handbook (PRH), Chapter 6. C. STAFFING The Professional will provide services, on Center, a minimum of twenty-four (24) hours per week at times and days mutually agreed upon by the Professional and the Center Director or designee, except during the two (2) week annual Christmas Break and July 4th periods. During holiday periods, appropriate on-call and referral procedures will be needed for dental emergencies. C. FINANCIAL CONSIDERATIONS 1. All funds providing payment for services rendered under this agreement will be furnished by the U.S. Department f Labor prime contract and all provisions and regulations of said prime contract are incorporated in this agreement by reference. 2. The Kittrell Job Corps Center agrees to provide payment for services rendered to enrolled Job Corps students, by the Professional at the rate specified in paragraph F., below for a maximum of twenty-four (24) hours per week for dental services to include dental hygienist services. 3. The Kittrell Job Corps Center agrees to provide payment for materials, supplies, drugs, x-rays, and laboratory fees procured by the Professional is the amount supported by adequate documentation on a monthly basis. 4. The Professional may provide more than twenty-four (24) hours of service per week (14 hours per week minimum), if deemed necessary for the provision of dental care to students but only with the expressed prior written approval of the Center Director and if the cost for the additional hours of service is within the maximum amount allotted to this agreement or any modification thereto. 5. The Professional assumes all financial liability for patient referral without the approval for the Center Director, or designee. 6. The Professional will provide a detailed monthly invoice in a format, which is compatible with Kittrell Job Corps Center financial reporting requirements.
