- Statement of Work for Payroll Processing Services for Off Duty Officers The Centers for Disease Control and Prevention (CDC) is requesting a quotation for Payroll Processing Services for Off-Duty DeKalb County Police Officers in Atlanta, Georgia. The Contractor shall provide all labor, tools, materials, equipment, and supervision necessary to perform electronic payment services for approximately 60 to 100 Off-Duty DeKalb County Police Officers on a bi-weekly basis. This contract is for a payroll processing service by which the Off Duty DeKalb County Police officers are paid. DUE DATES FOR QUESTIONS AND PROPOSALS: Questions are due NLT 5pm EST May 10, 2010 Responses to Questions will be sent to all offerors NLT May 13, 2010 Proposals are due NLT 5pm EST May 24, 2010 The Government intends to evaluate offers and award a contract without discussions and Offerors providing the requested service. Therefore, the initial offer should contain the offeror's best terms from a technical and price standpoint. Source Selection and Award will be made to the lowest price offeror whose proposal is rated technically acceptable. The North American Industry Classification (NAICS) Code is 541214 and the Size Standard is 500 employees for this requirement. All organizations may submit a proposal for this RFQ. However, the Government will evaluate proposals only from organizations that are certified by the U.S. Small Business Administration as small business concerns. If the Government's evaluation reveals that none of the small business concerns are technically acceptable, then the Government will evaluate proposals from all offerors (regardless of their size status). Each offeror needs to identify and certify its size status in the Representations and Certifications submitted with the quotation. Interested vendors capable of furnishing the Government with the services in this synopsis and Statement of Work attached should submit quotations to the email address listed below. The quotation shall include the itemized price, separate pricing for a 12 month base period and four (4) - 12 month option periods, prompt payment terms, the F.O.B. Point (Destination or Origin). Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNs), and descriptive literature of the product quoted. The quotation must reference Solicitation Number 2010-Q-12112. Quotations will be due on May 24, 2010 by 5pm Eastern Standard Time. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted via email to gmullins@cdc.gov. All interested parties that believes it can meet the above requirements, should submit a quotation. The quotation and any other furnished information must be in writing and must contain material in sufficient detail to allow CDC to determine if the party can fully meet the requirements herein. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors - Commercials Items (JUN 2008); 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (MAR 2009) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (APR 2010) applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (8), (9), (11), (12), (14), (15), (17), (18), (20) and (24). OFFERORS SHALL COMPLETE ONLY PARAGRAPH (b) OF 52.212-3 IF THE ANNUAL REPRESENTATIONS AND CERTIFICATIONS WERE COMPLETED ELECTRONICALLY AT http://orca.bpn.gov. IF THE REPRESENTATIONS AND CERTIFICATIONS WERE NOT COMPLETED ONLINE, THE OFFEROR SHALL COMPLETE A COPY OF THE PROVISION 52.212-3 (c) through (m), OFFERORS REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS, WITH YOUR QUOTE.
