Description
The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is seeking to obtain sources to assist with the provision of residential placement and supportive services through the Health Care for Homeless Veterans (HCHV) program. This contract is designed to serve homeless Veterans in need of immediate placement in a safe environment. Many of these veterans have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. Lebanon VAMC intends to engage a Contractor to provide rapid placement in a supportive, residential setting to VA referred Veterans The Contractor shall be expected to provide rapid residential placement that enhances the stability of Veterans and their families served by HCHV. Specific tasks are as follows: Task 1: Housing Available for Rapid Placement Contractor will be responsible for identifying sufficient residential capacity to place a minimum of 10 Veterans in safe, community-based residences. Identification of suitable residential placement generally requires: a) sufficient personnel to assure security; b) staffing available on-site 24 hours a day, 7 days per week; c) up to 3 healthy meals per day; d) available laundry facilities; e) the ability to place a Veteran on the same day of a referral from HCHV staff. Contractor will be responsible for planning and coordinating all activities associated with providing safe and appropriate residential care. This includes, but is not limited to, the elements described in the preceding paragraph. Any associated expenses should be broken out on a line item basis and included as "other direct costs" within the Contractor's Hybrid Fixed Price proposal. Task 1 Deliverables: The identification of a minimum of 10 residential beds specific to the needs of the HCHV program. Contractor is responsible for providing VA a written summary of size, location, and condition of the available residential beds. Task 2: Delivery of Support Services Contractor is expected to offer supportive services to Veterans that will provide assistance to them during their contract residential stay. These services will continue throughout their authorized stay, generally not to exceed 90 days. Therapeutic, rehabilitative, and recovery services determined to be needed by the individual resident in a plan developed by the contractor with consultation by the veteran and the VA case manager and/or other appropriate VA staff. Services which the contractor must be able to furnish include: a) Structured group activities as appropriate - examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities as appropriate; b) Collaboration with the VA program staff, which will provide supportive psychosocial services; c) Individual professional counseling, including counseling on self care skills, adaptive coping skills and, as appropriate, vocational rehabilitation counseling, in collaboration with VA program and community resources; d) Assistance to develop responsible living patterns and to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships; e) Support for an alcohol/drug abuse-free lifestyle; f) Assistance to gain and to apply knowledge of the illness/recovery process; g) The ability to assess and respond to a Veteran in crisis. Task 2 Deliverables: Contractor will provide a detailed staffing and service plan. The contractor will collaborate with the VA program staff, who will conduct treatment and discharge planning reflecting a team assessment of health, psychosocial and vocational needs and the involvement of residents' families and appropriate community resources in resolving problems and setting goals. Contractor is expected to provide HCHV staff with regular reports on changes in Veterans status, including all changes and observations concerning the Veterans biopsychosocial needs. Task 3: Status Reports Contractor shall be responsible for providing VA with documentation demonstrating that placement and support service objectives are being met throughout the contract period. Documentation includes compliance with paperwork required as a part of the VA's performance monitoring efforts. VA intends to conduct both periodic fiscal and performance reviewed of the selected Contractor. Task 3 Deliverables: The contractor shall make available to the VA, documentary information deemed necessary by the VA to conduct utilization review audits for the mandated national evaluation study as required by Section 2 of Public Law 100-6; to verify quality of patient care for veterans, to assure confidentiality of patient care for veterans, to assure confidentiality of patient record information, and to determine the completeness and accuracy of financial records. The contractor shall comply with the VA Patient's Bill of Rights as set forth in Section 17.34a, Title 38, Code of Federal Regulations. This is not a request for competitive quotes.
