- REP-SOW NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention, National Center for Emerging and Zoonotic Infectious Diseases (NCEZID), Atlanta, GA, hereby announces its intent to issue a purchase order to the Mayo Clinic, 200 First Street SW, Rochester, MN 55905 for services to access data of the Rochester Epidemiological Project (REP), which is an epidemiologic database containing comprehensive clinical records and to identify prevalent Chronic Fatigue Syndrome (CFS) cases from the REP database and assess patients for further evaluation in the Mayo Fatigue Clinic. This requirement will bring in a random sample of cases identified in the REP to the clinic.These persons will be assessed for current fatigue and metabolic syndrome status, current anxiety and depression will be evaluated and additional gynecologic/reproductive history will be collected. The REP database includes diagnostic codes for conditions identified by physicians at doctor's visits or hospital stays; a large proportion of patients have lifetime records included in the database. CFS occurs predominantly in middle age. We are interested in examining clinical events that occurred prior to the development of CFS, so it is important that we have access to a database that includes diagnostic information about childhood and adulthood clinical conditions. In addition to the database itself, REP can advise us on how to measure any clinical factors we are interested in examining as diagnostic factors, risk factors or causes of death using a combination of ICD codes and record abstraction. Using the REP to identify CFS cases and receiving patients to the Mayo Clinic for further examination allows us to look at CFS risk factors in a longitudinal design rather than cross-sectional. This project is supported by justification for other than full and open competition is to the Mayo Clinic under authority of 41 U.S.C., 253(c)(1). Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. Please forward the capability statement and pricing information to Maria Shamburger, Procurement and Grants Office, 2920 Brandywine Rd, Atlanta, GA 30341, Reference 2010-82246. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all capabilities statements and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.406-1(b)(1), negotiations will be conducted with the Mayo Clinic as the only source and a purchase order will be issued without any additional notices being posted.
