May 12, 2010 Department of Homeland Security Customs and Border Protection Request for Information For Medical Director 1.0 BACKGROUND A Market Survey is being conducted to determine if there are interested and qualified medical doctors to act as the Medical Director for the San Diego Sector Border Patrol's Emergency Medical Response Division's (EMRD) Paramedic certified agents. The EMRD consists of numerous EMT-Basics who are certified in San Diego County, and several EMT-Paramedics who are licensed in the State of California and accredited in San Diego County. The focus of this survey is to find a local physician to act as the medical director in order to provide direction for Advanced Life Support (ALS) medical services in the field. 2.0 REQUIREMENTS The Medical Director shall: 2.1 Be a licensed/Board Certified Physician and Surgeon in the State of California 2.2 Be assigned to the emergency department of a local base hospital and have admitting privileges. 2.3 Be trained to issue advice and instructions to pre-hospital emergency medical care personnel consistent with statewide guidelines established by the State of California Health & Safety Code, Division 2.5 2.4 Have five (5) years experience with an EMS System 2.5 Spend 1-2 days per week in the office/field with EMRD personnel 3.0 TASKS 3.1 General Tasks 3.1.1 Establishing and ensuring compliance with patient care standards, including medical aspects of communications, dispatch, progressive medical protocols, standing orders, and concurrent medical direction. 3.1.2 Ensuring appropriateness and initial qualification of EMTs, and other licensed or credentialed health care providers directly involved with the provision or supervision of out-of-hospital emergency care. 3.1.3 Ensuring maintenance of those skills and qualifications through oversight of continuing medical education, training, testing, and certification, recertification, and credentialing and licensing (as these apply to EMS). 3.1.4 Assisting the EMRD in the development and implementation of an effective QA/QC Program. 3.1.5 Participating in local, regional, and State activities to ensure standards, needs, and requirements are met, resource utilization is optimized and mutual aid, all hazards planning, hazardous materials and Chemical, Biological, Radiological, Nuclear, and Explosive incident (CBRNE) responses are medically appropriate. 3.1.6 Work in conjunction with the installation Public Affairs Officer to development and disseminate public education and information. 3.2 Authorities 3.2.1 Recommendation for certification, recertification, and authorization of out-of-hospital personnel, as appropriate. 3.2.2 Establishment, implementation, and revision of the clinical aspects of dispatch, triage, treatment, and transport of patients in the out- of-hospital setting. 3.2.3 Establishment of criteria for initial and graded response and transfer to ALS jurisdictional agency. 3.2.4 Establishment of clinical criteria and protocols for patient destination and patient-based or facility-based diversion. 3.2.5 Developing criteria for non-transport of patients. 3.2.6 Providing oversight and/or evaluation of EMS related education and testing for personnel involved with the EMS System, including: EMS dispatchers, First Responders, ambulance and other Emergency Medical Transport Vehicle (EMTV) personnel, including licensed non-EMS personnel who have occasion to provide clinical services as part of the EMS system, nurses and physicians involved with concurrent medical direction, EMS education coordinators and instructors. 3.2.7 Assuming medical oversight of the EMS QA/QC Program. 3.2.8 Administrative suspension or probation of EMS clinical privileges and revocation of authorization of non-credentialed clinical providers involved with direct or indirect patient care for cause, subject to due process. In the event that the involved party is a civilian employee, these processes shall be in accordance with applicable provisions of Title 5, U.S. Code. Recommendations concerning administrative suspension or probation of EMS clinical privileges and revocation of authorization may be made to the supervisory authority or reporting senior of credentialed clinical providers. Such action does not, in and of itself, automatically affect other clinical or professional privileges of the involved provider; however, the local authority must be advised of such action at the earliest possible date 4.0 PLACE OF PERFORMANCE 4.1 EMRD, Kincaid Road, Building 479, San Diego, CA 92101, and a new location for FY2011 to be determined within the limits of the city of San Diego. 5.0 PERIOD OF PERFORMANCE 5.1 The initial contract period will be for a base period of twelve (12) months from October 1, 2010 to September 30, 2011, plus five (5) option years. The U.S. Border Patrols intent under this requirement is to obtain the service of a qualified medical doctor to fulfill the tasks stated herein. 6.0 GENERAL INFORMATION 6.1 This is not a solicitation announcement and this notice does not constitute any commitment by Customs and Border Protection. The purpose of this Request for Information (RFI) is to solicit an interest from local medical doctors to provide oversight and medical direction. Any information received with responses may/or may not be used to develop a solicitation if needed. 6.2 All interested medical doctors should notify this office in writing by mail or email on or before May 20th, 2010, 5:00 pm Pacific Standard Time.
