- Amendment 0001. Amendment 0001 added 13 May 2010. The purpose of this amendment is to change the following: A) Block 7a of the SF 1449, Name, is changed from “Monty Bell” to “Mercedes Cook.” B) Block 7b of the SF 1449, Telephone Number is changed from “541-750-7114” to “503-668-1634.” C) Update FAR Clause 52.212-5 dated Apr 2010 to add check to FAR 52.219-14 Limitations on Subcontracting. D) Remove A-7(d) 2 through 6 (inclusive) from Description/Specifications/Statement of Work. E) NOTE: FAX and Email Responses will NOT be accepted. Hard copy offers must be returned to: USDA Forest Service EROC Northwest, 16400 Champion Way, Sandy OR 97055. F) All other terms and conditions remain unchanged and offerors must acknowledge all amendments to the solicitation.
