- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ).This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-41. NAICS Code for this requirement is 339112.The U.S. Naval Hospital, Okinawa, Japan intends to negotiate a sole-source contract with Stryker Corp under the Authority of FAR 13.106-1(b) However, any firm that believes it can meet the requirements may give written notification to the Point of Contact (POC). Offerors responding to this notice must be FDA approved for the manufacture, sale and distribution of these products. Offerors must be authorized distributors of Stryker. Contractor must be registered in Central Contractor Registration. [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award] Registration is available via the Internet at www.ccr.gov. Shipping shall be direct to U.S. Naval Hospital, Okinawa, Japan. The requirements includes the following schedule of supplies: Line Item 0001 5MM RATCHETED HANDLE, P/N: 250-080-234 - 8 EA - UNIT PRICE _________________, TOTAL PRICE___________________, Line Item 0002 SLIDING LOCK ATRAUMATIC GRASPER, P/N: 250-080-765 - 8 EA - UNIT PRICE _________________, TOTAL PRICE___________________, Line Item 0003 5MM PEEK MULTIFUNCTION HANDLE, P/N: 250-181-106 - 24 EA - UNIT PRICE _________________, TOTAL PRICE___________________, Line Item 0004 CUTTER F-SERIES 4.0MM TOMCAT, P/N: 375-545-000 - 12 BX - UNIT PRICE _________________, TOTAL PRICE___________________. Under the authority of FAR 13.106-1(b), sole source justification is based on Stryker. being the only company supplying these particular items which are continually utilized at USNH. Consumable work in conjunction with existing equipment and instrumentation.The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items and 52.212-4 Contract Terms and Conditions - Commercial Items. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. 1) Technical capability 2) Past performance 3) Price. Responses must be submitted to the POC no later than May 24, 2010; 12 PM; Japan Standard Time. Responses shall be submitted via e-mail in Word, Excel, or PDF format to the POC, Hiro Oshiro, at hiro.oshiro@med.navy.mil or faxed to 011-81-989-92-8575. Telephone inquiries or requests will not be accepted. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, U.S. Naval Hospital, Okinawa, Japan will proceed with the sole source procurement.
