Completed forms will need to be sent to the following address; EASTERN TRANSPORTATION CMT SOLICITATION NO. 150-201-10 1200 MERCANTILE LANE SUITE 109 LARGO MD 20774-5389 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than June 11, 2010 by 1500 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY.
