Documents needed for proposal The following information will be needed to complete forms that will be returned with your proposal; HCR # 45219 Solicitation # 150-215-10 Date of Solicitation – 05/17/2010 Begin Contract Date - 07/01/2010 End Contract Date - 06/30/2014 For Mail Service Between City, St – Cincinnati NDC, OH City, St – Greensboro NDC, NC Annual Hours – 14,167 Annual Miles – 597,025.4 Required Vehicle – (4) Tractor (12) 53’ Trailer Attachment labeled “Forms Prospective offeror must complete and return" will need the above information and be fully completed by the contractor. Completed forms will need to be sent to the following address; EASTERN TRANSPORTATION CMT SOLICITATION NO. 150-215-10 1200 MERCANTILE LANE SUITE 109 LARGO MD 20774-5389 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than June 14, 2010 by 1500 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY.
