- This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. The NAICS Code is 423450. The Department of Hunter Holmes McGuire Veterans Affairs Medical Center, Richmond, VA has a requirement for surgical staplers and reloads and intend to solicit quotes and award a firm fixed price contract. This is a Total Small Business Set Aside. Contractor shall provide the following equipment/supplies/Service: (1). Echelon Flex 60 stapler Item number, SC60A, Qty 04 BX. (2). Echelon reload 3.8 Item number, ERC60D, Qty 03 BX. (3). Echelon reload 4.1mm Item number, ERC60G, Qty 03 BX. (4). Echelon Reload 3.5, Item number ERC60B, Qty 02 BX. (5.) Endopath Flex-, Item number ATS-45, Qty 04 BA. (6). Endopath 45 Reload, Item number TR45G, Qty 02 BX. (7). Endopath Reload 3.5mm, Item number 6R$5B, Qty 02 BX. (8). Reload for ATW45, Item number, TR45W, Qty 3 BX. (9). ETS-Flex articulating Linear cutter 35mm, Item number ATW35, Qty 03 BX. (10). Reload 6 rows, Item number TSW35, Qty 03 BX. (11). The following provisions apply to this solicitation: 52.211-6 Brand Name or Equal, 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16)(23-24); Offerors are to submit pricing. Process of determining contract award: award will be to the lowest-priced responsive, responsible bidder who can provide the items. Warranty information shall be included in the proposal. All questions are to be submitted via email, response to questions received after May 24th 2010 will not be answered. All proposals must be submitted in written to the email address provided. This information should be specific, concise, and complete and provide sufficient information to demonstrate the offeror's capacity to meet or exceed the requirements of the contract.
