Description
The purpose of this amendment is to address the following question: 1. The specs only list the ultrasound but not a probe. Do you need a probe? ANSWER: Yes, a probe is required. The minimum specifications are amended to include the following: Probe: High Frequency Linear Probe, 6-15 mHz. The response date remains June 1, 2010 at 12:00 pm. The Uniformed Services University of the Health Sciences (USUHS), Bethesda, MD has a requirement for a portable ultrasound machine. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. It will be the responsibility of the contractor to check the website for the issuance of any amendments/changes to the solicitation. Quotes are being requested and a written solicitation will not be issued. This Request for Quotation (RFQ) incorporates all provisions and clauses in effect through Federal Acquisition Circular 05-40. The North American Industry Classification System (NAICS) code is 334510, Electromedical and Electrotherapeutic Apparatus Manufacturing, with a size standard of 750 employees. This proposed combined synopsis/solicitation is full and open. This RFQ is FOB Destination with delivery and acceptance at the Uniformed Services University of the Health Sciences. The Government requires the best commercial warranty offered to the general public and to the vendor's best customer. This requirement is for a firm fixed-priced purchase order for the following Contract Line Item Numbers (CLIN) outlined as follows: CLIN, Description, and Quantity. CLIN 0001 Ultrasound Machine Qty: 1 Point of Delivery: FOB Destination Location: Uniformed Services University of the Heath Sciences (USUHS) 4301 Jones Bridge Road Bethesda, MD 20814-4799 Minimum Specifications: The following requirements for the ultrasound machine are: The system must be field ready so it must be able to be transported with ease and must be sturdy to work in theater. Also, an ultrasound machine with pulse wave Doppler capabilities. The need for pulse-wave Doppler capabilities is needed to teach students to identify pneumothoraces on advanced Focused Abdominal Sonography for Trauma (FAST) exams and to do many vascular diagnostic exams that will be taught in the curriculum. Weight: 6 - 8lbs Dimensions: 10 - 12" L x 10 - 12" W x 3" H (small box) Display: 10 - 11" diagonal LCD (NTSC or PAL) Architecture: All-digital broadband multi-frequency imagining Connectivity 2 USB 2.0 ports Ethernet port Composite video out USB Storage Formats MPEG-4 (H.264), JPEG, BMP Compatible with Mac® and PC formats Remote Control Image Capabilities 2D imaging Tissue harmonic imaging Color Doppler optimization, Pulse Wave Doppler capabilities Auto Gain automatic image optimization Dynamic range and gain Duplex imaging 2x pan/zoom capability Probe High Frequency Linear Probe, 6-15 mHz Installation/Operations Instrument Customer training involves one or two days training on the basic system functionality: includes an overview of the system, setup, reporting, analysis, images, and system maintenance. The Government will award to the responsible offeror whose offer conforms to the combined synopsis/solicitation, is most advantageous to the government, based on technical, price, and delivery. Offerors must provide specifications and/or literature documentation of the ultrasound machine. To be eligible for contract award, a contractor must be registered with the Central Contractor Registration (CCR) database. For instructions on registering with the CCR, please see the CCR web site at http://www.ccr.gov. All offerors are required to submit a completed copy of the Federal Acquisition Regulations (FAR) Provision 52.212-3, Offeror Representations and Certifications - Commercial Item, with the offer.
