- Replacement Page New Drawings (Five Total). Amendment No. One (1) with New Specification. Amendment No. One (1) to the RFP-10-241-SOL-00048. This is a combined synopsis/solicitation for commercial items (Security Camera System Upgrade) prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Part 13, Simplified Acquisition Procedures, FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)), and FAR Part 15 Contracting by Negotiation. This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Aberdeen Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Proposal (RFP-10-241-SOL-00048). This is an Equipment and Service contract to remove and replace existing closed circuit TV (Security Camera) system and integrate new system with existing system in DDU at the Winnebago IHS Hospital, Winnebago, Nebraska. This solicitation is restricted to 100% Small Business concerns. The base year is from June 1, 2010 through September 30, 2010. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-39. The associated North American Industry Classification System code is 561621 and the small business size standard is $12.5 million. Offerors are requested to read the entire solicitation with drawings (20-total, suggest downloading and print on a printer with 11x17 capabilities), answer all criteria and fill out the bid schedule when submitting an offer. Please fill out and sign the SF-1449 along with all other basis of award instructions located in the commercial clauses section. PROPOSAL SUBMISSION INSTRUCTIONS: Please see commercial clauses section FAR 52.212-2 for all instructions to offerors. Additionally please submit a completed copy of FAR 52.212-3; small business representations and certifications or if you are registered with ORCA please submit this document with the offer. Offers shall be submitted to the Aberdeen Area Indian Health Service, 115 4th Avenue SE, Aberdeen, SD 57401 Attn: Craig Wells or email craig.wells@ihs.gov no later than 12:00 p.m., on May 28, 2010. The offer must be submitted in a sealed envelope, addressed to this office, showing the time specified for receipt, the solicitation number, and your Company name and address. ACCEPTANCE PERIOD: Your proposal must stipulate that it is predicated upon all the terms and conditions of this RFP. In addition, it must contain a statement to the effect that it is firm for a period of at least 90 days from the date of receipt by the Government.
