- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation RFQ number VA-246-10-RQ-0163 is issued as a request for quote (RFQ). No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offerors responsibility to ensure that the quote is received. The associated North American Industry Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 500. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. This action is unrestricted; small businesses, VOSB, SDVPOSB, women and small disadvantaged business are encouraged to submit offers. If offeror has a FSS/GSA number, please include it on your offer. The Department of Veterans Affairs, VISN 6 Centralized Acquisition Service has a requirement for a Mizuho OSI Jackson Table System which consists of the following components: 5803-Advanced Control Retractable Base; 5927-Radiolucent Imaging Top 500 lb Capacity; 5943AP-Spinal Top -500 lb Capacity with Advance Control Pad System and Modular Table Cart; 5356-Pivoting Arm Board with Mizuho OSI TemperMed Pad; 5840-820-Dual Chest Pads Set; 5808PV-Jackson Spinal Table Pkit w/Prone View Pillows 6/cs; 6977-959-Universal Side Rail Adaptor; 5888-Retractor Adapter for Jackson Spinal Table Top; 5321G-Wilson Plus RWF for JTS with ShearGuard Gel; 10604-Patient Safety Strap 96" with OrangeAID Gel; 5878PV-RWF for JTS Pkit w/Prone View Pillows 6/cs; 5979-1-Cervical Management Base Unit; 6950-6pc Tempur-Med OR Pad Set; 5996-12-ACP Quick Connect Tubing 12 sets/case; 6946-Tempur-Med Pillow for Sling; Include shipping, installation, and staff in-service in pricing. Item numbers referenced above are Mizuho Product numbers. This is a BRAND NAME OR EQUAL requirement. Or equal offers must include a full product description in sufficient technical detail to allow the Government to determine the item to be at least equal to the brand name items listed above. Delivery shall occur within 60 calendar days of contract award, shall be delivered to the VA Medical Center, 1100 Tunnel Road, Asheville, NC 28805. The contractor must indicate if there are freight charges. You must be registered in CCR to be considered for award, Central Contractor Registration, the offeror shall provide its DUNS, or if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offerors is registered in the CCR database. Lack of registration in the CCR database will make an offeror ineligible for requirements by calling 1-888-227-2423 or 269-961-5757, or via the Internet at http://www.ccr.gov. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, any shipping charges, company name, and point of contact, address, phone number, fax number, tax identification number, DUN number, and size status (i.e., small business disadvantaged business or woman owned business). Interested persons may identify their interest and submit a quote. The Government shall consider all proposals received by 2:00 PM Eastern Standard Time, June 4, 2010.
