- Performance Based Matrix Statement of Work This is a Centers for Disease Control and Prevention (CDC) combined synopsis/solicitation 2010-N-12113 for services that shall be provided by states due to the proprietary nature of the services. This announcement constitutes the only solicitation; proposals are being requested in accordance with the FAR Part 12 Clauses below. The Government contemplates award of a Firm Fixed Price contract with streamlined terms and conditions in accordance with FAR Part 12 and under authority of the Public Health Service Act. The data and records needed for this project are State owned, governed by State laws and regulations, and produced through State operated registration systems. This highly specialized data is available from no other source than the identified State or Territory. Each registration area has, by law, the exclusive authority to collect and record the events, and currently do so as a governmental function. As the sole entity authorized to and engaged in the collection of this information, each state agency is uniquely and solely capable of providing the requisite data to the Government. The NAICS Code is 541511. The Project title is Pilot Individual Early Hearing Detection and Intervention (iEHDI) Database for the Centers for Disease Control and Prevention (CDC), National Center on Birth and Developmental Disabilities (NCBDDD), Division of Human Development and Disability (DHDD), Early Hearing Detection and Intervention (EHDI) program. The purpose of this contract is to obtain a limited set of existing, individual level data from a minimum of six states. Information in a limited data set is not directly identifiable and excludes direct identifiers, such as names or social security numbers, of the individual or of relatives, employers, or household members of the individual. The data to be obtained is information that already exists and offeror's will not be required to collect new information, nor will they be required to develop a tracking system. The expected results are: *Individual level data files are provided to the CDC EHDI Team with a defined set of fields that represent four consecutive quarters of data. *Names and other specified identifiers have been removed and the data in the transmitted data files have successfully undergone pseudonymization. *Reported data files have been verified for completeness and accuracy by the contractor and CDC *Issues related to the preparation and transmission of the data files have been successfully resolved Individual level data is available for evaluation from the Pilot Individual Early Hearing Detection and Intervention (iEHDI) project by approved EHDI Team staff at CDC. Background and Need: The Centers for Disease Control and Prevention (CDC), National Center on Birth and Developmental Disabilities (NCBDDD), Division of Human Development and Disability (DHDD), Early Hearing Detection and Intervention (EHDI) program collaborates with federal, national, and state agencies and organizations in assisting states and territories to implement EHDI tracking and surveillance systems; assist states in enhancing EHDI infrastructure to improve their capacity to ensure that all infants are screened for hearing loss and receive appropriate follow-up services, as outlined in Goal 28-11 of Healthy People 2010; and maintain an information clearinghouse on issues related to EHDI that will be available through the CDC EHDI web site and teleconferences. Through these functions, the CDC EHDI program strives to ensure that newborns have the opportunity to communicate from birth. This program is a part of continuing national efforts to promote (1) early detection of hearing loss; (2) tracking of infants/children who are deaf or hard of hearing; and (3) the initiation of effective intervention systems. These efforts are essential roles of the CDC EHDI program. Part of the CDC EHDI program mission is to support states in the development of tracking and surveillance systems and collect and maintain a database with aggregate hearing screening and follow-up data. Although this aggregate level data has enabled the generation of national estimates of hearing screening, diagnosis and enrollment in intervention, there are several limitations and questions that cannot be addressed with this data, which is why more detailed, pseudonymized, individual data is needed. Please see the attached Statement of Work and Performance Matrix for the specific tasks and performance requirements. This will be a performance-based contract. The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation -- Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. 6. Additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices can be found in the attached SF 18 solicitation document.
