Description
1. Project Title: Request for information on data collection for improved hospital surveys 2. Purpose: This is a request for information only. It is not a request for proposal and does not commit the Government to issue a solicitation, make an award, or pay any costs associated with responding to this announcement. All submitted information shall remain with the Government and will not be returned. The National Center for Health Statistics (NCHS) health-care provider surveys include the National Hospital Discharge Survey (NHDS) and the National Hospital Ambulatory Medical Care Survey (NHAMCS). NHDS has, since 1965, provided critical information on the utilization of the nation's non-Federal hospitals and on the nature and treatment of illness among the hospitalized population. NHAMCS has provided data annually since 1992 concerning the nation's use of hospital emergency and outpatient departments. These data have been extensively used for monitoring changes and analyzing the types of care provided in the nation's hospitals. NCHS is planning to integrate the data we collect from these two hospital surveys into one survey called the National Hospital Care Survey (NHCS). Beginning in 2011, NCHS will recruit a new sample of hospitals for this new survey. During this first year the survey will replace the National Hospital Discharge Survey, and hospitals will be asked to provide data on all inpatients from their administrative databases of Uniform Bills (UB-04s). Then in 2013, the sampled hospitals will be asked to provide data on the utilization of health care provided in their emergency and outpatient departments (ED and OPD) and ambulatory surgery centers (ASCs), thus integrating the National Hospital Ambulatory Medical Care Surveys into the NHCS. The new NHCS will continue to provide the national general purpose health-care statistics that NHDS and NHAMCS currently provide. The new NHCS will have some distinct advantages. First, more information at the hospital level will be collected. Second, the survey will be able to sample hospital discharges with specific diagnoses and procedures for special studies that use medical record abstraction to collect more clinically relevant data. NCHS also plans to move toward collection of health-care data by electronic means as soon as hospitals participating in NHCS are able to provide such electronic data. In 2011 the UB-04s will be electronically transferred to NCHS. As hospitals adopt electronic medical records (EMRs), NCHS should be poised to accept electronic files from hospital medical records. Toward that end, NCHS is seeking input in two areas; you may respond to either one or both topics: A. Data collection instruments for the current NHAMCS include an induction interview questionnaire and patient record forms. All forms are paper. The induction interview is conducted in-person by a field representative (FR) from the U.S. Census Bureau. Data are abstracted from ED and OPD records and entered on the patient record form, pre-edited by the Census' National Processing Center, and sent to a contractor for data entry. Data collection instruments can be found at http://www.cdc.gov/nchs/ahcd/ahcd_survey_instruments.htm#nhamcs NCHS is interested in evaluating state-of-the-art data collection approaches that allow efficient and flexible data collection for both the induction interviews and patient record forms. NCHS invites interested organizations that have relevant expertise to describe improved method of data collection. Data collection approaches may be different for each form, and may include one or more methods. Data collection approaches may include but are not limited to one or more of the following: paper forms, web-based instruments, and computerized data entry on a laptop or other device. i. The approach to data collection must lead to an increase in the efficiency of the survey and improve the timeliness with which the data are released. ii. It should be indicated if the approach to collection improves data quality, thereby reducing the need for editing and funds required for data processing. iii. The data collection must be flexible, to allow additional data elements to be added with regularity and without undue redesign. iv. Hospitals that are participating currently in NHAMCS are given the choice as to whether they want to use the hospital staff to abstract the data or if they want the FR to abstract the data. The response to this RFI should indicate whether the suggested approach will permit this flexibility for the new survey. B. NCHS plans to move as quickly as possible to collecting clinical data from electronic medical records by electronic means. In 2011, this will include administrative data from UB-04s on hospital inpatients. Eventually, NCHS would like to collect data from electronic medical records as the hospitals move to electronic medical records. Hospitals have very different EMR capabilities. Some hospitals have limited EMR capabilities but have computerized laboratory records or electronic medication ordering and administrative records. Some hospitals have home grown computerized systems, and others use one or more of the many software venders. Not only are there many EMR/EHR vendors, but also a vendor typically customizes a system for each hospital. NCHS is interested in evaluating approaches to obtaining electronic data from health-care facilities. NCHS invites interested organizations that have relevant expertise in extracting data from electronic medical records to describe this expertise and how this could be applied to enhance the data collected by the National Hospital Care Survey. Response NCHS will use the information obtained from responses to this RFI to refine its approach to collecting data through the new National Hospital Care Survey. NCHS invites interested organizations that have relevant expertise to submit a written response to CDC. As a minimum, submitted information should include: a) A description of the respondent's approach to addressing one or both of the issues above; b) A description of the expected challenges in executing these approaches and achieving these objectives, and the manner in which the respondent's solution addresses these challenges; c) A description of the technologies required for the proposed approach; and d) The estimated costs of the proposed approach(es). REMINDER: This is not a request for proposals. Supporting cost data are neither required nor requested. Any cost data provided, including estimates, are informational only, and will be used to help formulate and refine the Government Cost Estimate for this requirement. This is an aspect of market research only. All information submitted to CDC will be kept confidential as allowed by relevant Federal law, including the Freedom of Information Act (5 USC 552) and the Trade Secrets Act (18 USC 1905). Information should be submitted by close of business June 30. Responses should be no more than 12 pages, in electronic format, and e-mailed to the attention of the technical contact person.
