Description
The Department of Veterans Affairs (VA), Veterans Affairs Medical Center (VAMC) -Iowa City, is soliciting and intends to award a firm fixed price contract for Architecture/Engineering (A/E) services, "A/E Services to Expand Clinics for Patient Centered Medical Home Model" at the VAMC in Iowa City, IA. NAICS Code 541310, size standard $4.5 million. The A/E contract will include but is not limited to all professional, technical and investigative services to prepare working drawings, cost estimates, specifications, and construction period services to design a multi-story addition to the west end of the main hospital building, redo the entrance to the urgent care ambulance garage, and demolish the existing chapel. Additionally, provide a planning study for expanding the Specialty clinics and adding a west end egress stair tower. Also included in this project is the demolition of the existing chapel. The first story of the addition will expand our primary care clinic for the Patient Centered Medical Home Model (PCMHM), upper levels will house Radiology, chapel, and conference space in support of Medical Home. All floors will be accessible from the existing main building. These are estimates only and may be adjusted as program criteria are finalized. Construction will require phasing to maintain access to occupied areas. In performance of this work, the A/E must include a Certified Industrial Hygienist (CIH) and an independent third party Fire Protection Engineer (FPE). The person assigned to these positions must be clearly indentified in Section E of the SF 330 in order for the A/E firm to be considered. The estimated construction cost range is between $5,000,000.00 and $10,000,000.00. Area of consideration: prime A/E firms must be within a 200 mile radius of the VAMC, 601 Highway 6 West, Iowa City, IA 52246. Please submit your qualifications and past performance data on the SF 330 (architecture-engineer qualifications); and for any consultants to be used, on the SF 330 sections E and F.
