Description
Performance Work Statement for Formulary Tool This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ FA4800-10-Q-BB03. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-41. It is the contractor's responsibility to be familiar with applicable clauses and provisions. (iv)This acquisition is 100% total set aside for Small Businesses. The NAICS code is 541511 with a size standard of $25 million. (v) 0001 - Pharmacy Formulary Communications Tool, Quantity 12, Unit of measure months (MO), Base year plus (4) Option Years (vi) Online formulary portal that will provide a full service set-up and ongoing support with a versatile, easy to use tool to manage, update and communicate the formulary in print, on the web, and on handhelds. For additional information, see the Performance Work Statement in the attachments section. (vii) The Period of Performance is 1 August 2010 to 31 July 2015, which includes 4 option years (viii) The provision at FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. (ix) The Government will award a firm-fixed price purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price factors considered. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable. (x) Offerors must complete annual representations and certifications electronically at the ORCA website to cover provisions at Clause 52.212-3, Offeror Representations and Certifications Commercial Items with its offer. The ORCA website is located at http://orca.bpn.gov. (xi) Clause 52.212-4, Contract Terms and Conditions - Commercial Items, is hereby incorporated by reference. (xii) Clause The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation, FAR 52.204-7, Central Contractor Registration, FAR 219-28, Post-Award Small Business Program Representation, FAR 52.222-3, Convict Labor, FAR 52.222-19, Child Labor-Cooperation with authorities and Remedies, 52.222-21, Prohibition of Segregated Facilities, FAR 52.222-26, Equal Opportunity, FAR 52.222-36, Affirmative Action for Workers with Disabilities, FAR 52.222-50, Combating Trafficking in Persons, FAR 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration, FAR 52.252-2, Clauses Incorporated by Reference, FAR 52.252-6, Authorized Deviations in Clauses. The clause at DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this solicitation. Specifically, the following DFARS clauses cited are applicable to this solicitation: 252.204-7004 Alternate A, Central Contractor Registration, 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports, 252.243-7001 Pricing of Contract Modifications, 252.246-7000 Material Inspection and Receiving Report. Addenda to FAR 52.212-5: The following additional clauses apply to this solicitation: FAR 52.233-2, Service of Protest: (a) Protest, as defined in section 33.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the General Accounting Office (GAO), shall be served on the Contracting Officer Cynthia Lewandowski, by obtaining written and dated acknowledgement of receipt from 633d Contracting Squadron, Attn: Ms. Cynthia Lewandowski, 74 Nealy Avenue, Langley AFB, VA 23665-2088. (b) The copy of any protest shall be received in the office designated above within one day of filing a protest with the GAO.
