- Contractor Inserts for review Purchase Order forms (to be completed) Business Associate Agreement Far Clauses Commercial Clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13.5, Test Program for Certain Commercial Items (10 U.S.C. 2304(g) & 2305 & 41 U.S.C. 253(g) & 253a & 253b), FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)), & FAR 37.4, Nonpersonal Health Care Service (10 U.S.C 2304 & 41 U.S.C. 253). This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Aberdeen Area Indian Health Service (IHS) intends to award a firm fixed-price commercial item contract in response to Request for Quotation (RFQ) RFQ-COM-10-168. This solicitation is restricted to 100% Small Business concerns. The solicitation documents & incorporated provisions & clauses are those in effect through Federal Acquisition Circular 2005-16. The associated North American Industry Classification System code is 621112 and the small business size standard is $10.0 million. PRICE SCHEDULE: 16 weeks (8 hours per week) = 128 hours @ $ ___________________ per hour = $________________________. The quoted "unit price" must be an all-inclusive cost. All "inclusive unit price" is defined to include travel, lodging, per diem, fringe benefits, federal, state and local taxes (contact the local Tribal TERO Office to see if taxes are applicable), plus all other costs pertinent to the performance of this purchase order. PURPOSE: The mission of the White Earth Health Center in Ogema, MN is to deliver quality care in order to attain positive health outcomes for all our valued patients with emphasis on cultural sensitivity and human dignity. To meet the mission, the White Earth Health Center requires a trained Psychiatrist to support and assist in temporary duties assigned at the clinic. Individuals providing services under contract, which involve regular contact with, or control over, Indian children, are subject to a character investigation in accordance with "Indian Child Protection and Family Violence Prevention Act" P.L. 101-630. The Indian Health Service Personnel Office will conduct these investigations following award of a contract unless in the judgment of the Contracting Officer, in consultation with the Personnel Officer, investigation may be waived based on prior investigations on file. Until the character investigation has been completed and the Contracting Officer notified, such individual should not have unsupervised contact with Indian children. The Contracting Officer has the authority to temporarily waive the character investigation of a contractor. The "Agreement to a Temporary Waiver of Character Investigation" authorizes an agency to hire a Contractor prior to the completion of a background check provided that at all times the Contractor is within the sight and under the supervision of a government employee who has successfully completed the national agency check with written inquiries (NACI). Contractor will be required to maintain suitable malpractice insurance and will not be covered under tort. The need for services is expected to remain constant during the term of the contract period. Therefore, the Contractor shall maintain the continuity of services to the maximum extent possible. Each contract resulting from this solicitation shall: a) be a fixed-price health care services contract under which the Contractor is subject to supervision and direction of a designated government officer; b) allow the government to evaluate the quality of professional and administrative services provided, but retain no control over the medical, professional aspects of the services rendered (e.g. professional judgments, diagnosis for specific medical treatments); and c) require the Contractor to ensure that its subcontracts for provision of health care services contain the requirements of the clause a far 52.237-7 (section i-4), including the maintenance of medical liability insurance. CONTRACTOR FURNISHED PROPERTY: The Indian Health Service does not anticipate a need for Contractor furnished equipment. GOVERNMENT FURNISHED PROPERTY, FACILITIES, AND SERVICES: The government will provide supportive pharmacy, scheduling and secretarial services. The contract Psychiatrist will be authorized to use all areas of the treatment facility available to civil mental health providers and commissioned personnel in similar positions. The same restrictions apply to the contractor Nurse Practitioner and any other Indian Health Service personnel to use areas for official business activities only. Each Psychiatrist shall be provided with an orientation to the Service Unit and the Indian community being served. Clinical personnel shall provide the orientation. The clinic shall provide all assessments and testing materials deemed necessary to provide psychiatric care at the clinic with the exception of those items to be supplied by the Contractor as listed. The clinic shall provide all required blank government forms applicable to the Indian Health Service. Clinical personnel will also provide appropriate orientation to the necessity of such forms. The clinic shall provide an ongoing method of monitoring and evaluation of the quality of services performed before the level of performance becomes unacceptable. Contractor will be under the direct supervision of Director of Mental Health. STATEMENT OF WORK: This position is located at the White Earth Health Center. It requires a board certified Psychiatrist who is approved to diagnose and provide medication management for a variety of patient mental health diagnoses. This position must be licensed in the State of Minnesota and approved for billing services. Preference will be given to Native American applicants and/or applicants who have experience working with Native American patients. This position is expected to treat three (3) scheduled patients per hour for a nine-hour work day. The Mental Health Department will provide support services for scheduling, mailing appointments and billing for sessions. Medication will be dispensed at the White Earth Health Center by Pharmacy staff. This position is under the supervision of the Clinical Director and Director of Mental Health at the White Earth Health Center. The White Earth Health Center is looking at up to a total of five (5) contract days per month. Patients to be treated are from17 years to 80 years old. The types of diagnoses may include adult bipolar disorder, depression, adult ADD, schizophrenic, PTSD, and anxiety disorders. RESPONSIBILITIES: The Contract Psychiatrist shall provide emergency and routine general psychiatric services for eligible patients (children, adults and elderly) at the White Earth Health Center. The Director of Mental Health will monitor contractor activity. Specific psychiatry services shall include, and are limited to the following: a) provides diagnoses and assessments of patients with a variety of mental health concerns, referrals of mental health patients to appropriate federal, state or private treatment facilities. The incumbent is responsible for selecting and implementing appropriate psychotropic medications for their diagnoses; b) provides on-going monitoring/changing of medications appropriate to patient experiences. Provider is responsible for prescribing all of the patient medications and to assess for allergies or allergic reactions; c) provides clarification, as requested, to pharmacy department; d) Provides consultation case by case, to other medical providers at the clinic, regarding psychotropic medications. Completes SOAP notes (or similar note-taking format) with appropriate medication directions using the current electronic health record patient encounter form; and e) maintains updated knowledge regarding special "black box" warnings and other concerns with specific patient medications. QUALIFICATIONS: 1) Graduate of a regionally accredited physician program with licensure as a MD, psychiatrist in the United States. 2) Must hold a current and unrestricted psychiatrist license from any of the United States. 3) Must possess documentation of continuing education. 4) Must apply for and have approved appropriate medical privileges by medical staff. 5) Must be able to bill for Services with MA, insurance or other third party provider. PERIOD OF PERFORMANCE: The contract Psychiatrist will provide services at the White Earth Health Center, Ogema, MN. Duty hours are nine (9) per day 8:00 am to 5:30 pm Monday through Friday with a ½ hour lunch break. Contractor shall work approximately one (1) day a week to be scheduled by the Director of Mental Health. The Director of Mental Health or designated representative may extend day schedule if work requires. Overtime hours are only to be used when absolutely necessary and approved by the project officer. Scheduled days will exclude recognized federal holidays: Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, New Years Day, M.L. King Day, Washington's Birthday, Memorial Day, Independence Day, and Labor Day. This is a firm fixed price all inclusive contract. The price proposed will include all Federal, State and Local taxes, per diem, travel and lodging expenses. Contract is for one (1) or up to three (3) days per week. This contract will terminate when the period of performance expires or if funds availability imposes constraints. REPORTING REQUIREMENTS: A detailed weekly invoice must be submitted to the Property & Supply Clerk or designee with proper signature authority no later than 12:00 pm on Monday of the following work week completed. Contractor will maintain a log of invoices and balance of remaining contract dollars. Invoice creation is the responsibility of the contractor, not the White Earth Mental Health support staff. The Director of Mental Health shall monitor the Psychiatrist's technical/post progress and promptly inform the Contracting Officer of any problems or potential problems they encounter. The government reserves the right to accept or reject services if the level of work is unsatisfactory. INVOICE SUBMISSION & PAYMENT: The Contractor shall submit a signed invoice to the Project Officer at the Hospital for approval on a least a monthly basis. The Contractor shall submit an original invoice to Accounts Payable, Aberdeen Area Indian Health Service, Division of Financial Management, 115 4th Ave SE, Room 309 Federal Building, Aberdeen, South Dakota 57401. The Contractor agrees to include the following information on each invoice: 1) Contractor's name, title, address and telephone number; 2) award number; 3) invoice number and date; 4) cost or price; 5) dates of service including number of hours worked; and 6) DUNS number and Tax Identifying Number (TIN). Payment shall be made by the Aberdeen Area Financial Management Branch, 115 4th Ave SE, Room 309 Federal Building, Aberdeen, South Dakota 57401. PROPOSAL SUBMISSION INSTRUCTIONS: All responsible offerors may submit a proposal, which shall be considered by the Agency.
