- U.S. Naval Hospital Okinawa Japan (NHO) has a requirement for REFERENCE LABORATORY SERVICES for a 12-month period beginning 1 October 2010 through 30 September 2011. Contractors must be certified to provide required services. The contractor shall furnish the necessary personnel, materials, equipment, and other services and expenses as may be required to provide services.This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice.This announcement constitutes the only solicitation; quotations are being requested. The North American Industry Classification System (NAICS) code is 621511, the SIC code is 8071. This acquisition is non-restricted.This solicitation and contract award is subject to availability of funds. Reference Number N6847011RQLC008 is issued as a request for quotation (RFQ)The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-41.The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items; 52.232-18 Availability Of Funds. The following DFARS clauses are applicable. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items; DFAR 252.204-7004, Required Central Contractor Registration. The contractor shall provide reference laboratory testing services in accordance with all applicable federal, state, and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, patient specimen testing for the specific range of referral testing within their capability. Services shall include the performance of analytical testing as defined by the Laboratory?s reference test manual, the reporting of analytical test results and consultative services as required to assimilate the full scope of its laboratory operations to NHO. All requirements and provisions defined in the specification of this solicitation will apply to any laboratory, i.e. branch, division, sub-contractor, etc. performing reference testing on behalf of the reference Laboratory. The Reference Laboratory shall provide laboratory services to include preanalytic processing as defined in its laboratory manual; analysis, reporting of analytic results, and interpretation. The Reference Laboratory must provide the following services: a. The contractor is responsible for providing requisition forms, special instructions, current list of tests with reference ranges and specimen requirements. b. Provide services during normal working hours unless otherwise specified according to special services provided by that Reference Laboratory. c. If applicable, transport samples in such a manner as to insure the integrity of the specimen. d. Analyze samples. The Reference Laboratory will comply with reporting test results within the specified time frame in accordance with the nature of testing, following all regulatory requirements for reporting of patient results. Test results shall be reported within the specified times established by the Reference Laboratories. Critical value test results shall be reported immediately by telephone or as otherwise indicated by NHO. Hard copy results will be faxed, mailed or sent via secure internet report, in compliance with HIPPA guidelines. e. Consult with laboratories on test result by telephone as needed. f. Provide the NHO with a means of communication to permit immediate inquiry regarding the status of pending tests. g. Reference Laboratory to receive customer generated work documents to order tests in lieu of individual requisition per request, if applicable. 1) The contractor shall provide a list of available testing services (specifically genetic testing, but inclusive of all tests) and fee schedule (price per test) information. The list and fee schedule will be used as part of the evaluation process. 2) NHO will be responsible for transport of specimens to the contractors site, but the contractor shall provide boxing and shipping material to NHO, as well as any special specimen collection supplies. 3) The type of tests and approximate number of tests will vary depending on NHO testing requirements for FY11. Below is a list of tests, with approximate quantities in parathesis. - Amniotic Fluid Chromosome Studies (30 tests) - InSight (Prenatal Inter FISH for Chromosomes 13, 18, 21, X and Y (25 tests) - Peripheral Blood routine Chromosome Analysis (25 tests) - Peripheral Blood Resolution Study (25 tests) - Peripheral Blood Mosaicism Study (4 tests/) - Tissue Culture for Non-neoplastic Disorders; lymphocyte (4 tests) - AFAFP (25 tests) - Sub-telomere Analysis (6 tests) - AChE (4 tests) - Fragile X Syndrome (30 tests) - Cystic Fibrosis 97 Mutation Analysis-Cfplus (6 tests) - Y-Chromosome Micro-deletion Analysis (4 tests) The Government will award a Firm-Fixed Price (FFP) contract resulting from this combined synopsis/solicitation to the responsible contractor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the services quoted to meet the Government requirement (including submission of descriptive literature); (2) Past Performance and (3) Price; all to be submitted with quotation. The Government reserves the right to award a contract on a multiple award basis. No telephone requests will be accepted, faxed and e-mail quotes will be accepted. Contractors are responsible for obtaining any and all amendments or additional information. Quotes and all pertinent data (descriptive literature, specifications, etc.) must be received by 22 June 2010 at 1600 (4:00 p.m.) Japan Standard Time (JST). Include company name, point of contact, address, phone number, Duns Number, CAGE Code, and Tax ID. As a basis for contract award, contractors must be registered with Central Contractor Registration (www.ccr.gov).
