Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number VA-256-10-RQ-0252 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-40. This is a solicitation is unrestricted under NAICS code 325413, the small business size standard is 500 employees. Contract line item numbers are as follows: Item 0001, Real Time Chain Reaction thermocycler (PCR Thermo cycler) used for influenza and H1N1 testing. Utilizes disc media to contain and process samples. Include all installation and setup cost. 1 year standard warranty to be provided from date of installation and be included in price. 1 Each Unit Price: $_________ Proposed delivery date: _________ The following are the Focus Diagnostics manufacturer system components (or equal*) 0001AA, ID NO. 76735 SIMPLEXA MICROFLUIDIC MOLECULAR SYSTEM'S INTEGRATED CYCLER F/ MOLECULAR LAB INTEGRATED CYCLER, 110V (MOL1001). 0001AB, MOL1200-LLAPTOP COMPUTER, Dell Latitude E5500 Laptop Windows XP Operating System 15.4 Inch Wide Screen LCD Memory: 4 GB Hard Drive: 160 GB 24X CD-RW/DVD Dell Wireless 802.11b/g 4 USB ports and software for cycler. 0001AC, Accessories box including; User manual, Ethernet cable, barcode scanner, mouse pad, universal disc loading block 2 each, applicator 2 each, laptop security cable. *Equal items may be offered; however, the burden is upon the vendor to provide adequate information included with their quotation to determine offered items to be equal to the items listed. ADDENDUM to FAR 52.212-1 Instructions to Offerors --- Commercial Items - Sub-Part 13.5 Test Program. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13 or 15 of the FAR or some combination thereof. A single award will result from this request for quotation. Offers are required to submit: 1) Pricing or Quotation. 2) A description of the items offered, the contractors assigned point of contact for the contract and/or quotation. 3) At least 3 references that the contractor currently or in the past 3 years has provided the same or similar products to; information to include; name, address, phone and/or email address. 4) Completed copy of 52.212-3, Offeror Representations and Certifications -- Commercial Items, unless it has completed its online certifications through the ORCA website. Failure to submit any of this information will result in an offeror being nonresponsive to the RFQ. Offers are due no later than 28 June 2010 to arrive NLT 4 P.M. Central Standard Time.
