The following information will be needed to complete forms that will be returned with your proposal; HCR # 788A9 Solicitation # 300-194-10 Date of Solicitation – 06/25/2010 Begin Contract Date – 08/01/2010 End Contract Date – 03/31/2012 For Mail Service Between City, St – Eagle Pass, TX City, St –Spofford, TX Annual Hours – 1,032 Annual Miles – 28,246.1 Attachment labeled " Forms Contractor must Complete and Return" will need the above information and be fully completed by the contractor. Completed forms will need to be sent to the following address; SOUTHERN TCMT SOLICITATION NO. 300-194-10 225 N HUMPHREYS BLVD STE 4126 MEMPHIS, TN 38166-7071 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than July 9, 2010 by 1500 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE.
