- Statement of Work This is a combined synopsis/solicitation number 2010-N-12146 for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. The Government contemplates award of a Firm Fixed Price commercial contract resulting from this Request for Proposal (RFP). Please review the attached Statement of Work carefully and provide your technical and price proposal accordingly. The associated NAICS code is 493120. The Period of Performance shall commence upon award of a contract and include a Base Year of 12 months and four Option Periods of 12 months. Your RFP response shall include: 1. A Technical Proposal that addresses the Technical Evaluation Criteria in the Statement of Work. Page limit of proposal is no more than 75 pages. 2. A Price Proposal that includes the individual pricing elements for the acquisition as related to the Statement of Work items. 3. Past Performance. Provide past performance information for the past three years that includes agency or company name, type of contract, amount and contact information. TITLE: Biologic Specimen Repository and Inventory Management for the Department of Health and Human Services (DHHS), Public Health Service (PHS), Centers for Disease Control and Prevention (CDC), National Center on Birth Defects and Developmental Disabilities (NCBDDD), Atlanta, Georgia The National Center on Birth Defects and Developmental Disabilities (NCBDDD) has a requirement for immediate and on-going storage and inventory management of biologic specimens collected under two projects: the National Birth Defects Prevention Study (NBDPS), and the Division of Blood Disorders' projects for the reduction and prevention of complications due to blood disorders. Objective: The overall objective is to reduce the morbidity and mortality of birth defects and blood disorders. We require ongoing contractor support services to help accomplish this objective. The specific purpose is to (1) transfer and store existing biologic specimens to contractor's facilities, (2) receive newly obtained specimens from CDC and grantees, (3) ship selected specimens to collaborators for data analysis, and (4) maintain an accurate inventory system of all biologic specimens. Scope of Work (Attached) - To provide NCBDDD projects representing on-going and long-term studies with a state-of-the-art specimen storage facility that includes sufficient storage capacity for future growth and a high quality inventory management system that allows timely entry and retrieval of specimens and maintains accurate historical records. The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation -- Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. 6. Additional contract requirement(s) or terms and conditions may be required for this acquisition if determined necessary by the contracting officer and consistent with customary commercial practices.
