Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation Number W912EE-10-T-0048 is being issued as a request for quotation (RFQ) with the intent to award as a simplified acquisition. The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2005-36. THIS PROCUREMENT IS COMPETED ON AN UNRISTRICTED BASIS AND ISSUED UNDER NAICS CODE 333923 (SIC 3531) WITH A SIZE STANDARD OF 500 EMPLOYEES FOR A FIRM FIXED PRICE CONTRACT. This procurement may be quoted as follows: Line Item 0001 Supplies/Services: Furnish, Deliver, and Install a Crane Load Moment Indicator System. Quantity: 1 Unit: Lump Sum, Unit Price $ ___________; Total amount for Line Item 0001 $____________. The U. S. Army Corps of Engineers, Vicksburg District has a requirement to furnish, deliver, and install a crane load moment indicator system. SCOPE. The contractor shall provide all plant, labor, equipment, materials, and supplies necessary to furnish and deliver one single drum spud winch. The spud winch shall be capable of raising a 36,000 pound 60 foot barge spud from the set position. The single wire line of the spud winch will be mounted on the deck adjacent to the spud well, routed through the spud well upper floating sheave, down to the spud lower fixed sheave, and back up to dead man at the top of the opposite side of the spud well thereby creating a two part line pull to lift the weight of the spud and overcome the frictional and suction forces associated with penetration of the spud in the set position. The spud winch shall have a drum capacity sufficient to lower the spud full depth. WINCH REQUIREMENTS.. The spud winch shall be similar and equal to Coastal Marine Equipment Model 1S52-137A4-1-19. The winch shall be single drum and sized for 150 of 1-3/8 diameter wire rope in 3 layers. The drum width shall be 19 inches or less and minimized as much as possible to reduce the horizontal movement of the upper floating spud well sheave. The winch shall be driven by a 480V 3-Phase electric motor through a fail safe electric brake and multi-reduction speed reducer developing 48,000 pounds line pull at 37 FPM (low Speed) and 24,000 pounds line pull at 74 FPM (high speed). The unit shall be fitted with a lever operated jaw clutch and hand wheel operated brake for manual controlled free fall operation. The unit shall be equipped with anti-bird nesting arrangement and manual lock-out for unpowered/unattended wire pay out. All electrical equipment, wiring, and motor shall be rated for use in harsh environment and all enclosures shall be stainless steel NEMA 4X. Any exposed rotating equipment shall be provided with full guarding to prevent injury. Reference Contract Clause 52.212-1 Instructions to Offerors--Commercial Items (APR 2008) (b) (10) and (11). The offeror shall submit a sufficient description contracts performed, or awarded within the past three (3) years, and supporting documentation of education, training, experience either in preventive medicine or in occupational health; which are relevant to the efforts required by the attached scope of work. The Contract Description shall include the following: a.Contract/Order Number and Award date. b.Contract/Order Total Dollar Value. Initial AND final or projected final, if contract is ongoing. Identify in specific detail for each contract listed, why or how you consider that effort relevant or similar to our effort. In determining relevancy, consideration should be given, but not limited to, such things as service similarity, size and complexity, contract type, contract environment, division of company proposing, and subcontractor interaction. 6. The contractor shall also provide the following supporting documentation at the time of quote submittal: a.Documentation to verify that each of the proposed physicians is a licensed medical doctor who is qualified to practice in the Vicksburg, Mississippi area. b.Documentation to verify that each of the proposed physicians is a graduate of a medical school approved by the Council on Medical Education on Hospitals of the American Medical Association. 7. Information obtained from the Point of Contacts identified in 6(e) will be used to evaluate offerors past performance. 8. If an offeror has no relevant past performance history, education, training, experience either in preventive medicine or in Occupational Health, the offeror must affirmatively state that it posses no relevant, directly related, or similar past performance. 9. Reference Contract Clause 52.212-2 EvaluationCommercial Items. The data provided will be used by the Government to evaluate and award one firm-fixed price service contract. 10.
