- Pricing Form This is a combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number AG-7604-S-10-0133 is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-38. This is a 100% set aside for small business concerns. The NAIC code is 334220 and the business size standard is 750. The Government intends to award a Firm Fixed Price Purchase Order. FOB Destination to USDA Forest Service Northeast Interagency Fire Cache, Grand Rapids, MN 55744. Required delivery is within 10-30 days after receipt of order. Anticipated award date is June 30, 2010. Items are to be procured under Simplified Acquisition Procedures (FAR 13.106-2). Offerors are required to include a completed copy of provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Quotes must include Solicitation Number, Business Size Certification, proposed delivery schedule, explanation of any deviation, payment terms, and be signed by an authorized company representative. Offeror must include Taxpayer Identification Number (TIN) and remittance address. All responsible sources may submit a quotation which shall be considered. Telephone requests will not be honored. Line Item 0001: Miscellaneous Communication Equipment installation parts (see Attachment I for Quote Form).The following FAR clauses and Provisions are incorporated by reference: 52.212-1; 52.212-4; 52.212-5 (52.203-6 Alt-1, 52.219-6, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35; 52.222-36, 52.222-37; 52.225-13, 52.232-33; 52.252-1 (fill in https://www.acquisition.gov/far/ ); 52.204-7. Request for Quotes is due June 28, 2010, 1400 hours, at US Forest Service, EROC Intermountain, 740 Simms Street, Golden CO 80401. Submit offer via e-mail, USPS, or common carrier.
