B.2 STATEMENT OF WORK 1. Title: Telecommunications Support. 2. Scope of Work: Contractor shall assist and provide on-site service at the VA Salt Lake City Health Care System (VASLCHCS) for all software applications for station design, telephone set up, software groups, trunking, networking, traffic and routing. The work to be performed will include, but will not be limited to minor software MAC (Moves, Adds, or Changes), VA Telephone and Phone Mail Training, Call Processing, Phone Mail applications and set-up. Support shall include holiday routing and recordings and maintenance of records of all telephone locations and switch information. Telecommunication support shall also be provided at off-site locations to include the Ogden Community Based Outreach Center (CBOC), Orem CBOC, West Valley CBOC and Main Street Mental Health Outpatient Clinic. 3. Background: The telecommunications equipment currently being used at the VASLCHCS campus and at off-site locations include the Siemens multi-node CBX 9751, Mod 30, HiPath 4000, 3800, and 150 telephone switches and Siemens multi-node Phone Mail. The Siemens Enterprise Network contract requires the system to be maintained by Siemens certified personnel to perform software changes, design, and configuration applications on a Siemens Telecommunication System. 4. Performance Period: Base Year with (4) four one-year options. Base Year: Date of Award through July 30, 2011 Option Year 1: July 31, 2011 through July 30, 2012 Option Year 2: July 31, 2012 through July 30, 2013 Option Year 3: July 31, 2013 through July 30, 2014 Option Year 4: July 31, 2014 through July 30, 2015 5. TERM OF THE CONTRACT This contract is effective one year from Date of Award plus four (4) Option Years. The Contractor shall perform no services after July 30, 2015 2. TOUR OF DUTY Monday through Friday, 7:30 a.m. to 4:00 p.m. Weekends and Federal holidays excluded. PLACE OF PERFORMANCE The work will be performed on site at the VASLCHCS in Salt Lake City, Utah. The contractor will provide remote support to the CBOC's located in Orem, Ogden, St. George, West Valley, Utah, and Pocatello, Idaho. Candidate may be required to travel to outpatient clinics as necessary to set up/train personnel, or troubleshoot problems on relevant telephone systems. 4. BADGE REQUIREMENTS As per Information Letter 90-01-6, Contractor Personnel Security Requirements. All contractors, subcontractors, and their employees will be required to provide two (2) forms of personal identification (list of acceptable ID is attached), fill out a Declaration of Federal Employment, OF 306, be fingerprinted and badged prior to starting work at the VA Salt Lake City Health Care System. The prime contractor shall submit a list of every individual that will be working on the VA campus and a copy of their Employment Eligibility Verification Form 1-9. If the individual will have access to VA patient records or computer systems, in addition to the steps above, they will be required to fill out a Questionnaire for Non-Sensitive Positions, Standard Form 85. This form is very extensive and the individual will require approximately 1-2 hours for completion. This form can be done PRIOR to reporting to Bldg. 4. There are two mandatory training classes; Providing a safe and Secure Environment for Health Care and Information Security and HIPA (Privacy Act), that will be required for each individual, also. These can be accessed at www.uchep.com. Please allow approximately 1 hour to complete the two modules. You will need to complete these two courses and print out the certificates of completion PRIOR to coming to the VA for badging. Each individual will be required to clear the VA and turn in their badge once the job is completed or they are no longer working at the VA. Final payment will be held until all paperwork and badges have been turned in. If a badge is lost, there will be a charge of $50 for each badge that will be deducted from the final payment.
