- Notice of Intent to Sole Source - Department of Veterans Affairs (VA) Veterans Health Administration (VHA) Veterans Integrated Service Network (VISN) 5 Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) Case Management in Rural Communities Procurement Office: Department of Veterans Affairs (VA) Veterans Health Administration (VHA) Veterans Integrated Service Network (VISN) 5, 849 International Drive, Suite 275, Linthicum, MD 21090 Response Date/Time: July 29, 2010 - 12:00PM EDT The VA VISN 5 intends to award a sole source contract to Verus Government Solutions under the authority of Federal Acquisition Regulation (FAR) 6.302-5 (Authorized by Statute) for the VISN 5 Collaboration with Rural Community Health Centers in support of the VISN 5 Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease Case (COPD) Management in Rural Communities Program. This project will provide focused care for patients with Congestive Heart Failure (CHF) and/or Chronic Obstructive Pulmonary Disease (COPD). This project will identify and provide skilled registered nurses (RNs) to serve as Case Managers for CHF and COPD patients. Implementing this pilot project would allow VISN 5 to gather information about the benefits of this program. Patients in rural areas receiving care from the following CBOCs will be included in this project: Cambridge, Charlotte Hall, Cumberland, and Stephens City. Notwithstanding, any firm that believes it is capable of meeting VISN 5's requirement, as stated herein, may submit their corporate capabilities, which if received by the response date of this announcement may be considered. Responses to this posting must be in writing and submitted to Donyale Smith, Contracting Officer, at donyale.smith@va.gov by the response date listed above. A determination not to compete the proposed requirement based upon the responses to this notice is solely within the discretion of the Government. (Note 22)
