This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W912L2-10-T-0019 is being issued as a Request for Quotation. This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-42. The associated NAICS codes is 721110, small business size $7M. COMMERCIAL ITEM DESCRIPTION: The USPFO for Arizona has a requirement for conference rooms and amenities and also Meals and Refreshment Breaks. Statement of Work SUNDAY September 26, 2010 Yellow Ribbon Program Family Preparation Activity, A Co 422 ITSB Phoenix, Arizona The Arizona Army and Air National Guard is in need of a conference center that can provide indoor meeting rooms for a total of 420 people in support of the Yellow Ribbon and Reintegration Program. This figure is an estimate of 40 children from the age of 0-5 years of age, 60 youth from the age of 6-17 years of age and a total of 330 adult attendees. Registration: "Needed from 7:00 am until 9:00 am are 6 six-foot clothed tables with two chairs at each table that will easily accommodate the registering of all adult attendees in a timely manner. These tables must have electric and internet connections easily accessible. Cost will be inclusive of ALL Taxes and Service Charges Table cost: ____________ Conference Room 1: "On Sunday, September 26th, 2010 from 6:00 am 4:00 pm, the conference center will provide one on-site conference room to accommodate 270 adults seated in a crescent round-type setting allowing all attendees to see and hear the presentations clearly, take notes, and enjoy breakfast and lunch at their seats. These tables will need to be clothed. The attendees will be returning to this room throughout the day, therefore this room must remain set in this way from 6:00 am to 4:00 pm. "Also needed for this room is a podium on a raised stage in the front of the room, audio/visual (projector, one large projection screen and an integrated speaker system) adequate for size of room. A wireless microphone will also be provided by the conference center. The sound needs to be heard clearly throughout the whole room. The attendees should be able to see the projected images well from wherever they are seated. All audio and visual components must be adaptable to standard laptop outputs. "Note pads and pens should be provided at each seat in this room. Cost will be inclusive of ALL Taxes and Service Charges. Room cost: _____________ Childcare Rooms: "On Sunday, September 26th, 2010 from 6:00 am 4:00 pm, needed are 5 conference rooms no smaller than 1,200 sq. ft. each; set with 5 tables and 8 chairs at each table. "In each of these 5 rooms the conference center will provide audio and visual (projector, screens and an integrated speaker system) that is adequate for size of room. All audio and visual components must be adaptable to standard laptop outputs. "Four total registration tables one each located just outside each of these rooms is required. "One of these childcare rooms must have high-speed internet connection. "These 5 rooms must be located together, near a bathroom and in a secure location; these rooms will be used for child care and youth activities. Cost will be inclusive of ALL Taxes and Service Charges. Room cost per room: ______________ Total cost: ______________ Standard Breakouts Rooms: "On Sunday, September 26th, 2010 from 6:00 am 4:00 pm, 6 breakout rooms set in classroom style or pods to accommodate approximately 35 adult attendees in each room. "These 6 breakout rooms must be in the same general area as the conference room 1 to allow for easy transition to and from each breakout. The conference center will provide audio visual (projector, screens and an integrated speaker system) that is adequate for size of room, a table in the front of the room, and flip chart easel with pad and markers or a dry erase board in each room. All audio and visual components must be adaptable to standard laptop outputs. Cost will be inclusive of ALL Taxes and Service Charges. Room cost per room: ______________ Total cost: ______________ Large Breakout Room: "On Sunday, September 26th, 2010 from 6:00 am 4:00 pm, 1 breakout rooms set in classroom style or pods to accommodate approximately 70 adult attendees in each room. "This breakout room must be in the same general area as the rest of the conference rooms to allow for easy transition to and from each breakout. The conference center will provide audio visual (projector, screens and an integrated speaker system) that is adequate for size of room and table in the front of the room. All audio and visual components must be adaptable to standard laptop outputs. Cost will be inclusive of ALL Taxes and Service Charges. Room cost per room: ______________ Total cost: ______________ Vendor Support Room: "On Sunday, September 26th, 2010 from 7:00 am 4:00 pm the conference center will provide 30 clothed tables (preferably 6-foot rectangles) with two chairs per table in a room no smaller than 3,000 sq. ft. "This room must be indoors and within the general vicinity of Conference Room 1 and the breakout rooms. "This room will need to have outlets to electricity every twenty feet along at least two walls. "There are no A/V requirements for this room. Cost will be inclusive of ALL Taxes and Service Charges Total Room and Table cost: ___________ Counseling Room: "On Sunday, September 26th, 2010 from 7:00 am 4:00 pm the conference center will provide one small conference room to be used for counseling. One round table and six chairs are needed in this room. No A/V or internet is required for this conference room. Cost will be inclusive of ALL Taxes and Service Charges Total Counseling Room cost: ___________ Meals and Refreshments: Sufficient number of buffet lines must be provided to allow all attendees to access the breakfast and lunch in a timely manner. Breakfast "On Sunday, September 26th, 2010 from 6:30 am 8:30 am the conference center will provide a continuous and refreshed continental-style breakfast (such items as muffins, fruit, pastries, juices and coffee etc&) for the 430 attendees to enjoy after they register. The attendees will need sufficient area to enjoy this breakfast with their children, therefore, there will need to additional seating for breakfast, in addition to the Conference Room 1 that will be set for just 270. Adult Lunch "At 11:30am and again at 12:30pm a two-entre hot lunch buffet with a variety of sides, salad, soup, bread and two choices of dessert for a total of 330 total adult attendees will be provided. Child Lunch "40 box lunches will be delivered to the childrens (0-5 years of age) rooms at 11:00am. Venue must consider the various ages of the children and their abilities when preparing the boxed lunches. For example peanut butter and jelly, chicken tenders, french fries, macaroni and cheese, and a brownie or cookie. The children will be provided juice, water and milk. The boxed lunch needs to provide a second dessert to enjoy in the afternoon session, such as a bag of animal crackers, pudding snack or a cookie. Youth Lunch "At 11:00am a hot lunch buffet for the youth (ages 6-17) will be provided a choice of two-entres, a variety of sides, salad, bread and two choices of dessert for 60 youth will be provided. This buffet must include a menu that will cater to this age group, such as chicken tenders, french fries, macaroni and cheese, pizza, prepared fruits, salads and two desert options. The 60 youth will require an indoor area in which to eat lunch, separate from the adult lunch area. Beverages "Throughout the day the conference center will provide beverages (soda, iced tea, coffee, water etc&) accessible for 320 adult attendees to enjoy as they walk to each of their breakout sessions. This will eliminate the need for pitchers of water and glasses needing to be refreshed after each breakout. The 100 children and youth will be provided milk, water and juice as needed throughout the day. Vendors must bid these beverages keeping in mind we cannot forecast actual consumption. Vendors should bid at an amount they are comfortable with no matter the consumption. Contractors must attempt to bid this line item keeping in mind the Government Per Diem rate (per meal) in your area. Invoices shall reflect a breakdown for the per meal charges for the three groups of attendees, children, youth and adults.
