Description
THIS IS A COMBINED SYNOPSIS / SOLICITATION NOTICE. This is a combined synopsis / solicitation for commercial items, prepared in accordance with the format of FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference solicitation number is: NHLBI-PB-(HL)-2010-231/DDC. This solicitation is issued as a Request for Quotation (RFQ). This acquisition is being conducted under FAR Part 13, simplified acquisition procedures and the resultant purchase order will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 05-44. The total contracted dollar amount, including options, if any will not exceed $5 million. The acquisition is being conducted as a competitive Small Business Set-Aside. The North American Classification System (NAICS) code applicable to this requirement is 423450 and the associated small business size standard is 100 employees. The delivery point is the National Institutes of Health, National Heart, Lung, and Blood Institute (NHLBI), Laboratory Animal Medicine and Surgery (LAMS) Bethesda, Maryland. Market research conducted by the Government has determined that similar Microscopes are manufactured in sufficient quantities by at least three (3) manufacturers which confirm the commercial market. Also the market research conducted has determined that there are at least three (3) small businesses that are able to supply the required equipment The National Institutes of Health (NIH), National Heart, Lung, and Blood Institute (NHLBI), Office of Acquisition (OA), intends to award a separated purchase order for the below listed products. Major tasks required of the Contractor include the following: Government Specifications for Microscope The Government=s anticipated delivery date is 9/30/2010. The microscope with accessories must have the following minimum essential characteristics: - A minimum of 1:6 zoom system with zoom and focus motorized and integrated into the microscope body. - Must be able to perform motorized manipulation by the way of Polar and Planar XY function. - Must have capability to interchange components with our existing Zeiss (OPMI CS-XY on an S-4 Floor Stand) surgical microscope for ease of use and maximum economy. - Shall have fiberoptic light guide illumination with xenon superlux 300 high intensity illumination with built in halogen back-up. - Shall have built in, motorized, button activated, focus that allows focusing from 200mm to 400mm. - Floorstand shall have backup zoom control, as well as tilt capability to provide emergency backup of control functions. - Microscope body shall have position memory function to return to preset user settings. - Must have adjustable stereo binoculars positioned 180 degrees from each other, permitting two surgeons to see exactly the same image with each surgeon being equidistant from the subject. - Must have an additional optical port that is par focal with surgeon's binoculars and allows for video feed to be captured for documentation. The provisions of FAR Clause 52.212-1, Instructions to Offerors-Commercial Items is applicable to this acquisition. The provisions of FAR Clause 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. The purchase order award will be made to the Offeror whose equipment meets the Government=s minimum performance specifications, price and delivery date above.. The Offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The addendum to FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, is as follows: The Offeror must submit a quote for the product, and must include in its quote, the unit price, the list price, installation charges, shipping and handling cost, the delivery date, prompt payment discount terms, the F.O.B. point (Destination or Origin), and provide descriptive literature inclusive of specifications as part of the quotation. The DUN and Bradstreet Number (DUNs), the Taxpayer Identification Number (TIN), and the business size. The Offeror must be registered in the Government=s Central Contractor Registry (CCR) System, which is available at www.ccr.gov, The FAR Clause 52.212-5, Contract Terms and Conditions Required Implementing Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions, applies to this acquisition. The following FAR clauses cited are applicable to this acquisition: (1) 52.222-26, Equal Opportunity (E.O. 11246).
