- Scope of Work-Montreal, QC, CA Apartment The awarded contractor is intended to provide the United States Coast Guard Sector Buffalo, NY, a partially furnished executive style apartment within a 1 mile radius of the Transport Canada Marine Security satellite office in the Hochelaga District of Montreal, Quebec, Canada. The apartment must be up to code and energy efficient to the satisfaction of the U.S. Government. The term of the lease shall for one year with an option for renewal at the end of each year, not to exceed two option years. Provide a base year estimated cost as well as option year one estimated cost and option year two estimated cost. The contractor shall provide all supplies and services detailed as stated in the following. A. The contractor shall provide a partially furnished 2-3 bedroom apartment withan office space to include gas/electric, cable or satellite television, hot and cold water, sewage, trash, snow removal, lawn care and all applicable taxes, if any. Offers shall breakdown the cost between rent and utilities, with utilities estimated for a twelve month period. The suite shall also include smoke detectors in every room (either hard wired or battery) and the following amenities: Living Room: (1) Two end tables with table lamps (2) One coffee table Kitchen: (1) Sink with hot and cold water (2) Gas or electric stove/range (3) Refrigerator (4) Dishwasher (5) Microwave oven (6) All cooking utensils, i.e. pots, pans, strainer, spatula, stirring spoon, whisk, knives, measuring cups/spoons, baking pans, cookie sheets, cutting board and water pitcher (7) Table setting to include plates, bowls and silverware for four people (8) Trash and recycling receptacles. Dining Area/Room: (1) Dining room table with four chairs Bathroom: (1) Shower with curtain and liner or frosted shatterproof door (2) Flush able toilet (3) Wash basin and vanity mirror/medicine cabinet (4) Trash receptacle Bedrooms: (1) Twin bed (either bunk beds or stand alone) (2) One dresser (3) One night stand with lamp (4) One alarm clock Laundry: (1) Laundry facilities shall be made available on site Office Space: (1) Broadband Internet (2) Lockable door Parking: (1) One on site parking space shall be provided, currently possess street parking permits for two Government vehicles B. The awarded contractor shall also provide cleaning service once a week between the hours of 9am-5pm. The scope of the cleaning services rendered will include the following: Living Room: (1) Dust (2) Vacuum Kitchen: (1) Sweep and mop floors with disinfectant (2) Wipe down all counter tops/table tops andsink disinfectant (3) Wipe down toilet with disinfectant (4) Clean all mirrors (5) Sweep and mop floors with disinfectant (6) Empty trash receptacles and leave three additional trash bags Bedrooms: (1) Dust (2) Vacuum C. The awarded contractor shall also provide for a s semi-annual cleaning service to be provided between the hours of 9am-5pm. The scope of the cleaning services rendered will include the following: All spaces: (1) Remove all scuff marks on floors (2) Clean all carpets, contractor will have to move all furniture necessary to facilitate cleaning (3) Clean all inside windows and window coverings (4) Remove all obvious marks on the walls below eye level (5) Vacuum all fabric on furniture (6) Clean stove and refrigerator interior and exterior surfaces D. The awarded contractor shall not be permitted to enter the office computer space as it may contain SSI or FOUO materials. The space shall be secured to the best extent possible while the apartment is vacant as a result of crew change out or mission accomplishment. The cleaning anbd maintenance of this space shall be the sole responsibility of the Coast Guard members using the apartment. E. Payment to be made monthly in arrears by the USCG FINCEN, P.O. Box 4115, Chesapeake, VA 23327-4115. Vendor must be registered in CCR or be willing to register at www.ccr.gov. Payment will be made by electronic check. Rent shall be prorated for a lesser period as 1/30th of the per month rate F. The utility rate is based on an average monthly cost calculated over a twelve month period. Twelve (12) months after initial or subsequent occupancy, this lease may be adjusted upward or downward to compensate for incorrect estimates for utility amounts. It is the Lessor's responsibility to monitor utility consumption and to provide the Coast Guard with verifiable utility consumption documentation for a twelve (12) month period. Failure to provide required documentation will not justify utility payment adjustments G. It is the Lessor's responsibility to maintain all utility machinery in efficient operation. The Lessor is responsible for notifying the Coast Guard immediately of any excessive utility costs. Documentation supporting utility abuse must be provided by the Lessor prior to any reimbursement processes or authorizations. H. The Coast Guard may terminate this lease at any time by giving at least 30 days notice in writing to the Lessor. Said notices shall be computed commencing with the day after the date of mailing. No rental payment shall accrue after the effective date of termination. All above services and supplies shall be made available for the term of the rental as set forth by the Contracting Officer's purchase document and the rental agreement.
