- Performance Plan Performance Work Statement Solicitation document for Ambulatory Accreditation Survey services. The Air Force District of Washington (AFDW) A&KM-S intends to issue a solicitation for non-personal services for Ambulatory Accreditation Survey Services at Brooks City Base, San Antonio, Texas. The Contractor shall provide all personnel, equipment, tools, supervision, and other items and services necessary to ensure that Ambulatory Accreditation Survey services is performed at both CONUS and OCONUS Air Force Military Treatment Facilities (MFTs) totalling 63 MTFs in a manner that will maintain accreditation in accordance with the Performance Work Statement (PWS). The contractor shall accomplish all accreditation service tasks identified in the PWS, in order to meet the requirements and the Service Summary (SS). These documents will be provided at time of solicitation. All work performed by the contractor shall be performed in accordance with all applicable laws, regulations, standards, instructions, and commercial practices. This solicitation is full and open competition. The North American Industry Classification System is 541990. The Standard Industrial Classification Code is 8742 and the small business size standard is $7M. The period of performance will be a five (5) month base plus four (4) option years. The approximate performance start date is 1 Aug 2010. The total contract period will not exceed four (4) years eleven (11) months. This solicitation is a competitive RFP and award will be based on commercial procedures found in the Federal Acquisition Regulation (FAR) Part 12. Award will be made using Lowest Price Technically Acceptable in accordance with FAR Part 15. The solicitation will be issued on or about 1 Jul 2010 and will be available to view and download on the Federal Business Opportunities (FBO) website: http://www.fbo.gov/. PAPER COPIES OF THIS SOLICITATION WILL NOT BE MAILED. Be sure to check the solicitation after issuance to verify the correct proposal response date. All amendments will be posted on the FBO website as well the date and time for the pre-proposal site visit.
