- This is a combined synopsis/solicitation for the purchase of commercial items, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. Solicitation number VA-260-10-RQ-0986 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44. The relevant NAICS Code is 339112 with a corresponding size standard of 500 employees. TELEPHONIC REQUESTS FOR THIS SOLICITATION WILL NOT BE ACCEPTED. The description of the commercial item is as follows: CLIN 0001: 3 each - System 6 SAG Saw, P/N 6208-000-000. CLIN 0002: 3 each - Dual Trigger Rotary, P/N 6205-000-000. CLIN 0003: 3 each-System 6 RECIP, P/N 6206-000-000. CLIN 0004: 3 each - Synthes Attachment Small, P/N 6203-110-000. CLIN 0005: 3 each - 5 ¼ inch Chuck Key, P/N 6203-131-000. CLIN 0006: 3 each - Hudson/Modified Trinkle Attachment, P/N 6203-135-000. CLIN 0007: 3 each - Adjustable Pin Collets, (2.0-3.2 MM) P/N 6203-126-000. CLIN 0008: 3 each - System 6 Large Sterilization Case, P/N 6102-450-000. Vendor quotes shall include all freight charges. Deliveries of all items are desired no later than 30 days after receipt of order, FOB Destination. The ship to address is: Department of Veterans Affairs, VA Puget Sound Healthcare System, 1660 South Columbian Way, Seattle, WA, 98101. The shipment must be clearly marked with the applicable purchase order number. This is a Brand Name or Equal purchase for the System 6 SAG Saw and components. If quoting a brand-name equal item, please include the description with specifications of the item you are quoting. The Contracting Officer will evaluate 'equal' items based on the information provided by the Quoter or identified in the quote and easily available for the Contracting Officer to review. It is the responsibility of the Quoter to provide information demonstrating that the offered equivalent item(s) meet or exceed all salient features, characteristics, and dimensions of the brand-name item(s). The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. Unless the Quoter clearly indicates within their quote that the product being offered is a brand-name equal item, the Quoter shall provide the brand-name item identified in this solicitation. The selection resulting from this request for quotations will be made on the basis of technical (go/no go) and the lowest-priced quote from a responsible Quoter with acceptable or neutral past performance. Once the Contracting Officer has decided to whom an order will be issued, a purchase order will be issued using a SF 1449. The provision at FAR 52.211-6, Brand Name or Equal, applies to this acquisition. FAR 52.212-1, Instructions to Offerors Commercial (JAN 2004), applies to this acquisition. In accordance with 52.212-3, Representations and Certifications, offers must either be accompanied by Representations and Certifications or the offeror must be currently registered in ORCA at website https://orca.bpn.gov/. The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (OCT 2003) applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders --Commercial Items (Jul 2010) applies, The following clauses within FAR clause 52.212-5 apply: 52.219-28, 52.222-3, 52.222-19, 52.222-36, 52.225-1, 52.225-13, and 52.232-34. VAAR clause 852.203-70 applies.
