- DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Organization: Department of Veterans Affairs, VISN 06, Salem Medical Center 2.Description of Action: This is a request for other than full and open competition for the procurement of off -site Anesthesia Services for Emergent Cardiovascular Surgical back up coverage for the Cardiovascular Cath Lab during Cardiac Interventional Procedures from Valley Anesthesia. 3.Description of Supplies or Services: Specific services needed are those that relate to off -site Anesthesia Services for Emergent Cardiovascular Surgical back up coverage for the Cardiovascular Cath Lab during Cardiac Interventional Procedures. In providing Anesthesia services while working with the Lewis-Gale Hospital staff during Emergent Cardiovascular Surgical Services. The requirement period of performance of performance is from 10/1/2010 through 9/30/2011 with four (4) one year option periods. 4.Statutory Authority: The rationale for the use of a limited source for this procurement is FAR Subpart 6.302-1(a)(2) - other than full and open competition is justified when services may be deemed to be available from only one responsible source. ( X ) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements; ( ) (2) Unusual and Compelling Urgency; ( ) (3) Industrial Mobilization, Engineering, Developmental or Research Capability, or Expert Services; ( ) (4) International Agreement ( ) (5) Authorized or Required by Statute; ( ) (6) National Security; or ( ) (7) Public Interest 5.Validity of Use of Statutory Authority: These services shall be performed at Lewis-Gale medical facilities. The incumbent Valley Anesthesia is currently privileged and have provided the Anesthesia services since 2005. The contracted provider much within 10 minutes of Salem VAMC and be currently privileged to practice at Lewis-Gale medical facility. Emergent Cardiovascular Surgical care expertise is not reliably present at the Salem facility, and there is no VA infrastructure in place to provide for development and rapid dissemination of expertise for Emergent Cardiovascular Surgical Services. The Service are outsourced to Lewis-Gale Hospital. There are no comparable resources that have privileging and have the capacity and history of success in effective providing Anesthesia Services Emergent Cardiovascular Surgical Services 6.Actions to Promote Competition: An intent to sole source notice was published in FedBizOps on July 12, 2010 in an attempt to offer any other potential sources an opportunity to express interest in the requirement. 7.Determination of Fair and Reasonable Price: The anticipated price to the Government is anticipated to be cost per case of $3,500.00 for the base year, $3,750.00 estimated for Option years 1 thru 4 for a total estimated contract cost of $16,500.00 will be considered fair and reasonable if it is no more than what is charged to the general public. If price reasonableness cannot be determined, negotiations will be conducted. 8.Market Survey (FAR 10): Market research was conducted on June 16, 2010 in accordance to FAR part 10, using the terms Anesthesia Services. A search of vendors listed Salem, Virginia vicinity under NACIS 622110 General Medical and Surgical Hospitals, $34.5m Training Services resulted in 0 potential vendors. A search of gsa.gov resulted in 0 potential vendors, both offering limited print and online palliative care and palliative medicine training. A search of vetbiz.gov resulted in 0 potential vendors. A search of google.com resulted in the following potential vendors: None of the potential vendors have the current Privileges to practice at Lewis -Gale Hospital. 9.Other Considerations: For this requirement we have used every tool available to try to identify alternate sources. None exist that can provide the comprehensive Anesthesia Services that meet VAMC's needs in this field. This requirement is critical to patient care.. 10. Listing of Sources that expressed, in writing, an interest in the acquisition: No other firm responded expressed interest in this requirement. 11.Statement of Actions to Overcome Barriers to Competition: At this time there are no actions to be taken to overcome barriers to competition as Valley Anesthesia has been determined to be the only qualified source available to perform the services. 12.Approvals in accordance with FAR 6.304 a.Contracting Officer's Certification: (required) I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. _____________________________ ________________________ Name Robert Headen, JR.Date Title Contracting Officer Facility VISN6 b.
