- COMBINED SYNOPSIS/SOLICITATION (REPLENISHMENT, TRAINING, INSPECTION FOR FIRST AID KIT) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only notice; a written solicitation will not be issued. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This procurement is in support of Defense Media Activity (DMC) at 23755 Z Street Riverside CA 92518. Solicitation/synopsis reference number 2165-0209-0521 is being issued as a Request for Quotations (RFQ) and incorporates the following with the word "quoter" substituted for "offeror". Provisions and clauses are those in effect through Federal Acquisition Circular 2005-44, dated 8 Jul 2010 DFARS change 20100723. (1) FAR 52.212-1 (Instructions to Offerors - Commercial Items) (JUN 2008): 20090913, dated 21 Jul 09 (a) Re para (a): NAICS 812199; this requirement is unrestricted, Standard 7 Millions. (b) Note that para (k) requires quoter to be registered with the Central Contractor Registration (CCR) database at time order is issued and throughout performance. (2) FAR 52.212-2 (Evaluation - Commercial Items) (JAN 1999) with the following insertion in para (a) "lowest priced technically acceptable quote with acceptable past performance." Technical evaluation will be whether quoted items satisfy all requirements in CLIN 0001 to CLIN 0005 below. (3) QUOTATIONS MUST BE RECEIVED NOT LATER THAN 2:00 PM PACIFIC TIME 20 August, 2010 IN ORDER TO BE CONSIDERED. Quotations may be emailed to Milagros.Halstead or sent via fax to (951) 413-2432. Due to technical issues email and fax may not result in your proposal arriving before the cut-off time, please allow extra time if you're sending your quote via electronic means. (4) Quotations must include completed copy of provision at FAR 52.212-3 (Offeror Representations and Certifications - Commercial Items) (AUG 2009) with Alt 1 (APR 2002) or indication that reps and certs are in ORCA. CLIN 0001 Description: Base First Aid Kits Inspect and replenish first aid kits through out the compound quarterly. POP 1 September 2010 to 31 August 2011. See Attached See PWS. 1 Job Unit Price Total Amount CLIN 0002 Description: Option First Aid Kits Inspect and replenish first aid kits through out the POP 1 September 2011 to 31 August 2012. See Attached See PWS. 1 Job Unit Price Total Amount CLIN 0003 Description: Option First Aid Kits Inspect and replenish first aid kits through out the POP 1 September 2012 to 31 August 2013. See Attached See PWS. 1 Job Unit Price Total Amount CLIN 0004 Description: Option First Aid Kits Inspect and replenish first aid kits through out the POP 1 September 2013 to 31 August 2015. See Attached See PWS. 1 Job Unit Price Total Amount CLIN 0005 Description: Option Provide Initial One Time Training to support for the requirement. 1 Job Unit Price Total Amount POC: George Taylor or Mike Dodge Facility Manager 951-413-2352 The Government will issue an order to the responsible offeror with acceptable past performance, submitting the lowest priced technically acceptable quotation.
