Description
NOTICE OF SOLICITATION Special Envelopes-Brown Kraft Envelopes The Social Security Administration has a requirement for 2- 9x12 window brown Kraft Envelopes. The windows are specially placed and must be exact to ensure that the inserts are visible to the U.S. Post Office. These envelopes shall be delivered to the Social Security Administration located at 300 Spring Garden Street, Philadelphia PA 19123. The delivery shall be dock to dock and must be by appointment only. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations, Subpart 12.6 and the procedures in FAR Part 13, as supplemented with additional information included in this notice. This accouncement constitutes the only solicitation; quotations are being requested and a written solictiation shall not be issued. Socicitation Number SSA-RFQ-R03-10-1015 and is issued as a Request for Quote(RFQ). The solicitation document and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. All responsible business sources may submt an offer which shall be considered by the Agency. This acquisition is a small business set aside. The NAICS code is 424120. Requirement Description: The windows on these envelopes are not a standard positioning and must accommodate the insert, which shall be provided to the vendor. It is imperative that the window and envelope specifications be exact to ensure that the recipient's name and full address are visible. These envelopes are used for beneficiaries and physicians responding to Agency requests for information. One of the envelopes is a business reply envelope and the new postal requirements for the Intelligent Bar Code must be implemented. The second envelope is a brown, Kraft envelope with a portrait position printing and window as opposed to the standard landscape print and window position. All items are to be shipped FOB Destination. The following items are required. Imprinted Brown Kraft poly window, Business Reply Envelope 9 x12 -sample with insert available. a. Envelope 9x12, 24lb Brown Kraft envelope with poly window positioned at bottom/off center of the envelope. Landscape printing. b. Business Reply Set up using the new postal intelligent bar code requirements. c. Window positioning critical to the Agency. The poly window shall allow the post office to see the insert name and fully address. d. These envelopes shall be hand-stuffed. No mail equipment will be used. Proof shall be required prior to the mass production of this envelope Dock to Dock delivery.- FOB Destination -Appointment is required. Envelope # 2 - Quantity- 75 MX, Price per thousand_______ Extended Price________ 2. Imprinted Brown, Kraft poly Window Envelope- 9 x 12-sample with insert available. Envelope is in Portrait position with printing in left hand corner above poly window. Window is located near top of envelope and to the left. a. imprinted 9x 12 poly window envelope-24lb, Brown Kraft envelope sample with insert available. b. Window positioning critical to the Agency. The poly window shall allow the post office to see the insert name and fully address. c. These envelopes shall be hand-stuffed. No mail equipment will be used. d. Self-Adhesive closure for envelope flap. Shipment is Dock to Dock.- FOB Destination- Appointment is required Proof is required prior to the mass production of the envelope. The clauses at FAR Part 52.212-4, Contract Terms and Conditions-Commercial Items(JUN2010) applies to this acquisition. The clauses at 52.212-5, Contract Terms and Conditions Required to Implement Statutesor Executive Orders-Commercial Items(JUN2010) applies to this acquisition and the additional FAR clauses cited in the the clause applicable to the acquisition: (b)(8),(19),(20),(21). INSTRUCTIONS TO VENDORS- The following provisions at FAR 52. 212-1, Instructions to Offerors(JUN2010) shall apply to this acquisition The Government shall award a firm,fixed price purchase order. All quotations shall clearly marked with the solicitation number SSA-RFQ-R03-10-1015 and shall contain the following: 1. Price per thousand and the extended price(FOB Destination) for each line item. 2. Complete description of the envelope including size. 3. Supporting evidence to provide and test the applicable intelligent bar code. This evidence can be self-certifying, but the bar code must able to independently pass the postal service requirements. 4. Certifications and Representations 5. DUNS Number and Tax Identification Number Shipping Information: The successful vendor shall deliver the required envelopes within the 30-45 days after award to the following address: Social Security Administration 300 Spring Garden Street Philadelphia PA 19123 Attention: Gene Shirley The operations are located North of Center City Philadelphia with public parking and public transportation operating on both sides of the building. An early morning appointment, between 6-7 AM, is required for Tractor Trailer Deliveries. Entrance for Tractor-Trailer deliveries are limited to 13'6" height and must be made using the 3rd Street side of the building. No appointment is required for non-tractor-trailer deliveries. Standard box trucks may deliver between 8 AM and 2 PM, Monday through Friday. May use 4th Street side of building. Height restriction is 13'2". PAYMENT: Payment shall be made by Government Purchase Card- No exceptions shall be made. EVALUATION: To provisions at FAR Part 52.212-2, Evaluation, Commercial Items are applicable to this acquisition. Award shall be based on the lowest priced technically acceptable quotation. REPRESENTATIONS AND CERTIFICATIONS: Quoters shall complete the annual representations and certifications electronically via the ORCA website at https://orca.bpn.gov. If the vendor has already filed his representations and certifications under ORCA, then he must certify that submission of these online certifications and representations are accurate and complete and in accordance with FAR Part 52.212-3 have been entered, and uploaded within the last 12 months. All quotes shall be submitted electronically no later than the posted due date and time to the following email address. Vendors shall include all of the required items or the quote shall not be accepted. An inquiry was made concerning the Mail ID form required by the Post Office to implement the Intelligent Bar Code. This form will be provided by the successful vendor to the Agency. It would not be fair to burden all individuals responding to the RFQ. Please note that the quantities are 75, 000 of each envelope. An additional Requirements have been added to the description: It is a mandatory that both envelopes shall contain at a minimum 20-30% postconsumer recycled content. This is in accordance with Executive Order 13101. Additional Shipping Information. A copy of the enclosed envelope shall be affixed to each carton. End of Amendment. August 5, 2010, Responses to Inquiries: 1. Removal of the following statement from Description. This was incorrectly included. This acquisition is not expected to be less than $12,000.00 2.
