Description
Floor Plans Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z3840141A005; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-43 2 Aug 10, Defense DCN 20100810, and AFAC 2010-0402 20100402. The North American Industry Classification System (NAICS) code is 314110. The business size standard is 500 employees. The Federal Supply Class (FSC) is 7220. The Standard Industrial Classification (SIC) is 2273. THIS IS 100% SMALL BUSINESS SET-ASIDE. Travis AFB intends to award a contract for the removal, replace, and installation of vinyl flooring in Bldg 543 Vet Clinic in accordance with the attached Statement of Work and the below items. CLIN 0001: PREP EXISTING FLOOR TO RECEIVE OVERLAY FLOORING 2800 Square Feet (SF) CLIN 0002: CHANGE SLOPE ON HALLWAY NEAR EXTERIOR DOOR 300 Square Feet (SF) CLIN 0003: PREP BATHROOMS 365 Square Feet (SF) CLIN 0004: PROVIDE/INSTALL ETERNAL WOOD SHEET VINYL WITH HEAT WELD SEAMS AND INSTALL WITH INTEGRAL 6" COVE FOR 2 OPERATING ROOMS, 2 EXAM RMS AND 2 BATHROOMS 135 Square Yard (SY) CLIN 0005: PROVIDE/INSTALL SHEET VINYL HEAT WELD SEAMS ON HALLWAYS/OFFICES 240 Square Yard (SY) CLIN 0006: PROVIDE/INSTALL FLOCKED TILES TO USE IN OFFICES (WARMTH OF CARPET,CLEANS LIKE VINYL) 420 Square Feet (SF) CLIN 0007: INSTALL TRANSITION BETWEEN VINYL AND FLOTEX FLOCKED TILES 24 Linear Ft (LF) Please fill out the last two pages of the statement of work and also submit your specs. Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition.
